Family Focused Therapeutic Case Manager

Supervisor : Family Focused Therapeutic Foster Care Manager- Wichita

Position Function : The Family Focused Therapeutic Case Manager (FFTCM) is the practical leader of the treatment team. The FFTCM has the primary day-to-day responsibility for leadership of the treatment team. The FFTCM organizes and manages all team meetings. Provides ongoing supportive case management of therapeutic services to children placed in the program which includes service planning, E-Cans administration, coordination of services, case documentation, crisis management and on-call support, foster home support, supervision, and on-going development. The FFTCM will respond to all referrals and coordinate admissions for assigned homes and will support homes and clients to stabilize placements in the least restrictive setting. The FFTCM ensures clients receive all available services and are prepared for their permanency plan which may include family reunification, adoption, or transition to independent living. The job requires sensitivity to the service population’s cultural and socioeconomic characteristics.

Education Requirements : A Master’s degree in social work, psychology, or related human services field from an accredited college or university is preferred along with a license from a state licensing body in Social Work or psychology or a related human service field. If not licensed upon employment, you must be able to sit for the exam within 6 months of employment and obtain the licensure. OR A Bachelor’s degree in Social work, psychology, or related human services field from an accredited college or university is preferred along with a license from a state licensing body in Social Work or psychology or a related human service field. If not licensed upon employment, you must be able to sit for the exam within 6 months of employment and obtain the licensure.

Experience Requirements:

The position requires two (2) years of cumulative experience with a graduate degree or five (5) years of cumulative experience with an undergraduate degree which includes working with youth providing crisis intervention, intake and assessment, and foster care case management services. Understanding of crisis intervention, trauma informed care interventions, and minimum standards related to working with adolescents and families is preferred. The position requires good verbal and written communication skills. The candidate must possess a working knowledge of basic computer skills.

Functional Requirements: Requirements include providing effective therapeutic services to clients and families; be present for all placement of clients in assigned Foster Homes, provide ongoing assessment and service

Family Focused Therapeutic CM Page 1 of 4 coordination for clients, facilitate POS meetings to develop and document preliminary and ongoing service plans, case management, transition planning, and provide timely and accurate case documentations for all clients and assigned families. Work as part of a multi-disciplinary team. Provide on call 24/7 for emergency with families and children on assigned caseload. Coordinate the collaboration of referral agencies, family members, and other support systems to meet the needs of youth. Prepare and submit all required documentation, reports, and statistical data in a timely manner. Follow policies and procedures of ACH and represent the agency in a positive and professional manner.

Working Conditions: Exposure to children with behavior problems, including possible physical aggression. Exposure to illness and blood. A personal, dependable vehicle is required for travel.

Exposure to Confidential Information: Maintain confidentiality & follow policies related to personnel, foster family, & client records.

Key Expectations/Responsibilities:

Programming:  Provide 24/7/365 on call support for all foster families and clients on assigned caseload.  Facilitate and document service planning for all assigned clients and work as part of a multi- disciplinary team.  Complete all paperwork accurately and within required time frames  Visit the home at least once weekly or more often in times of crisis  Develop relationships with youths teachers, counselors and administrators at schools, attend court proceedings, and attend therapy or psychiatrist visits as needed to maintain open communication and effectively engage in assigned cases.  Provide expedient and effective case management services and therapeutic interventions to support assigned families and clients.  Provide skills training for assigned families or clients, when appropriate  Implement group programming to address the social and emotional needs of clients relating to achievement of individual goals.  Facilitate coordination with TDFPS caseworkers, families, community and mainstream support resources.  Maintain compliance with all contracts, licensing and accreditation standards, and ACH policies and procedures.  Document and assess progress of each assigned client  Maintain 40 hours annually of TBRI, and other trainings of high risk or challenging behaviors of youth as approved (i.e. .Emotional disorders, MHMR).

Community Relations  Build and maintain positive working relationships with CASA, TDFPS, OCOK, therapists, School, Courts, and other community partners as needed for service delivery.

Documentation and Reporting  Maintain up-to-date and accurate case records.  Facilitate, develop, and document preliminary, initial plan of service, and plan of service reviews.  Provide regular case management services for each assigned client  Communication and coordination of needs for each client  Communicate goals of the service plan and guide implementation of the plan with the direct care team  Assist in development of program related improvement processes and evaluation tools.  Develop, review, and update processes for service planning as needed.  Review all discharge data for timely and accurate entry into the client management system.

Other Duties Assigned  Attend and actively participate in team meetings  Attend designated agency or community meetings or training.  Complete trainings and certifications as required by licensing standards or agency procedures.  Assist with other agency program’s activities as assigned.  Work with Recruitment team to help identify families for program.  Assist with fund raising or other agency functions as needed.  Other applicable duties as assigned by supervisor.
