SHASTA COLLEGE – EOPS/CARE & TRiO SSS SPRING 2016 ACADEMIC PROGRESS REPORT

STUDENTS:

For CARE and TRIO, this progress report is mandatory. It is designed to help you evaluate your success. It must be submitted to the EOPS and/or TRiO Office by 5:00 pm March 11, 2016.

For EOPS students, you may choose to attend an EOPS Success Seminar or do a Progress Report to fulfill the 2nd contact requirement. However, your academic progress in all classes must be EXCELLENT or SATISFACTORY in order for this report to count as your 2nd Contact. If an instructor indicates that your progress is UNSATISFACTORY, you will need to make a counseling appointment at the time you turn in your progress report. Deadline for 2nd contact counseling appointments is March 25, 2016.

Distance education students can mail (must be post marked by the due date) the form or submit it to one of the other campuses by the due date. What is the purpose of the Academic Progress Report? The progress report allows you to connect with your instructors to see how you are progressing and allows EOPS and/or TRiO to identify students who will benefit from academic assistance such as:  Follow-up counseling to develop or revise your education plan  Referrals to relevant on and off campus resources such as tutoring, study skills, etc.

Tips for approaching your instructors:  Stop by your instructors’ office during their office hours  Make an appointment to see them  Catch them after class, if they are available  Email them, if the option is available  Don’t ask your instructors to fill out your progress report right before class is starting  Don’t leave the progress report with your instructors  Don’t procrastinate by waiting to do your progress report at the last minute

For Late Starting Classes: Instructor’s signature is needed if you have attended the class for at least two weeks.
