To My Patients:

I want to let all my patients be aware of our practice guidelines regarding missed or broken appointments. You will receive a courtesy reminder call from our office 48 hours prior to your scheduled appointment. It is with concern that with busier lives and schedules today that we are all dealing with, that this notice will give you time to make any necessary changes. Your oral health is of utmost concern to us. Please understand that this time is reserved especially for you. We do understand that emergencies occur.

Our guidelines are as follows:

1. Following a courtesy call of 48 hours and you do not keep your scheduled appointment, a fee of $25 per unit of time scheduled will be charged to you. For example, an hour would be charged $100.00. 2. If you cancel or reschedule your appointment less than 48 hours in advance, a fee of $25 per unit of time scheduled will be charged to you. 3. Pre-paid scheduled appointments for large cases will forfeit $25 per unit of time scheduled.

This notification is for your convenience. I know that very few of you will be affected by these guidelines since almost all of you either keep your appointments or call to reschedule well in advance. Please note that this charge will be billed to you directly and is not covered by your health insurance benefit.

My dental team will make every effort to provide a convenient appointment time for you which suits your work and personal schedules. We thank you for your understanding our practice guidelines.
