COEUR D’ALENE HIGH SCHOOL PARENT-TEACHER ASSOCIATION (PTA)

INSTRUCTIONS AND APPLICATION FOR 2015 PTA SENIOR SCHOLARSHIP

Each year the Coeur d’Alene High School PTA awards a limited number of senior scholarships derived from the proceeds of the Coeur d’Alene Craft Faire fundraiser. To qualify for the PTA General Scholarship applicants must demonstrate a combination of academic achievement (G.P.A.), leadership, and involvement in the community through volunteer service, and/or participation in extracurricular activities including work experience. The Jan Feely Volunteer Scholarship focuses on a commitment and excellence to community and volunteer service, in addition to the above general scholarship criteria.

Each scholarship award recipient will be asked to attend the Senior Awards Assembly to be held on May 27, 2015 in the CHS Auditorium. The scholarship award will be paid directly to the scholarship recipient’s educational institution of choice. The recipients must furnish their educational institution information and social security number to the CHS Counselor’s Office to facilitate the payment of the awards. A letter informing the college/university/trade school of the award and remittance will be sent upon verification of completed student registration. All monies shall be returned to the CHS PTA if the student does not attend school within the 2015-2016 school year.

INSTRUCTIONS AND REQUIRED DOCUMENTATION

1. Please type or write out your responses on separate pages, print one copy, sign the CHS PTA Senior Scholarship Application form and attach the below described required documents. You may use additional pages as necessary. All information contained in the application is confidential and will not be released to any other party; however, the names of each winner will be posted on the school’s website. Applications are limited to seniors graduating from Coeur d’Alene High School. By signing this application you are verifying that your statements are accurate and your intent to pursue and complete a certified course of study at a higher educational institution. Note: Incomplete applications will NOT be accepted.

2. Obtain an OFFICIAL, STAMPED, SEALED, 7th Semester, Coeur d’Alene High School Transcript (see Lori Humphries, Counseling Secretary) and attach it to the Application. Please allow 7-10 days to process your request.

3. Obtain Letters of Recommendation – Attach two (2) letters of recommendation from individuals who are not your family members (i.e., an individual who knows about your academic background, leadership, extra-curricular activities, work experience and volunteer work service hours).

4. Submit your complete Senior Scholarship Application with attached required documentation to Lori Humphries in the CHS Counselor’s Office. Deadline to submit your application is Friday, May 1, 2015 at 3:00 p.m. Note: Late applications will NOT be accepted.
