The California Integrated Waste Management Board (CIWMB) has received a request for a copy of the attendance list used for the Mandatory Site Visit held on October 16, 2002 for this RFQ process. Enclosed is a copy of the site visit attendance list (Attachment A).

In addition, the following pages of the RFQ have been modified and are attached for your reference. Deletions are identified by strikeout. Please replace the identified pages in your RFQ package with the attached modified pages identified in the upper right hand corner with “REVISED 10/22/02”.

 Completion Checklist for Tracy Tire Fire Site Remediation Services Contract Request for Qualifications;  Section I, Introduction, Subsection F, Requirements Of Statement Of Qualifications (page 2); and  Section VIII, Statement of Qualifications, Subsection B, Licensing/Health & Safety Information (page SOQ-4).

E. MANDATORY SITE VISIT

Attendance and participation in the site visit held on October 15, 2002 at 10:00 a.m. is mandatory to any firm wishing to participate in this RFQ process. A representative from the potential Proposer must be present at the site visit or the potential Proposer will be deemed non-responsive and subsequently disqualified from the RFQ process. During the site visit only site details we be discussed. All contract questions and issues must be submitted in writing in accordance with Section II, Subsection H, Inquiries of this RFQ.

The site visit will start promptly at 10:00 am. The Tracy Tire Fire Site is located at 29425 South MacArthur Drive in Tracy, San Joaquin County, California. Various driving directions and a map are provided for your reference in Attachment H.

F. REQUIREMENTS OF STATEMENT OF QUALIFICATIONS

A cover letter must be included with the SOQ package and must be signed by an individual who is authorized to contractually bind the Proposer. The cover letter must be printed on the business letterhead and contain the following information:

1. Name and address of Proposer; 2. Name, telephone number and e-mail address of a contact person; and 3. Name, title, address, telephone number and e-mail address of the individual(s) with authority to execute a binding contract on behalf of Proposer.

The following documentation and forms must be completed, included with your SOQ package and received by the Board by the specified due date and time specified:
