Dear Applicant,

I am pleased to inform you that on the basis of result of AlPMT-2014 held on 04/05/2014 for the admission to MBBS/BDS course and your registration for Institute of Medical Sciences, Banaras Hindu University for the said course, you are required to appear in person for the counselling on 08-08-2014 in the office of the undersigned. In case you fail to present yourself before admission committee on above date, your case for admission will not be considered subsequently. Merely attending the counselling does not guarantee a seat. Your admission is subject to the availability of the seats and following conditions: 1. That you have qualified the AIPMT-2014 examination. 2. That you have registered in the IMS-BHU MBBS/BDS admission for session 2014-2015. 3. That you have passed the qualifying examination i.e. 10+2 or equivalent. 4. That you have passed medical fitness tests assessed by Medical Board of the Institute of Medical Sciences, BHU. 5. Applicant has to appear in person before admission committee of the Institute at 8.00 AM on 08-08-2014 in the office of the Director, Institute of Medical Sciences, BHU. The counselling would start after the verification of the documents. The medical examination by the Medical Board will follow this. 6. Please bring ORIGINAL Certificates and Mark sheets of: a. High school or its equivalent, which shall be retained by the Institute. b. Intermediate (10+2) or its equivalent which shall be retained by the Institute. c. Qualifying Score Card of AIPMT-2014 d. School or college leaving certificate/Transfer/Migration certificate from institution last attended. e. Two Xeroxed copies each of all certificates/Mark sheets and AIPMT-2014 admit card. f. Eight copies of your coloured passport size (3.5X 4.5cm) photograph similar to that you pasted in application form of AIPMT-2014/ IMS BHU-2014 registration. g. A character certificate from the Head of the Institution last attended/ Gazetted Officer. h. Admit Card of AIPMT-2014 i. Rank letter of AIPMT-2014

Varanasi 221005, UP, INDIA T : 0542-2367568; 2307500 F : (91) 0542-2367568 E: [email protected] W: www.bhu.ac.in/ims HOWEVER, THE CANDIDATES HAVING OFFICIAL CERTIFICATES IN ORIGINAL FROM THE CONCERNED VICE CHANCELLOR /DIRECTOR /DEAN/PRINCIPAL OF THE UNIVERSITY/INSTITUTE/COLLEGE WHERE THE CANDIDATE IS ADMITTED AND HAS DEPOSITED HIS/HER ORIGINAL CERTIFICATES WILL ALSO BE ALLOWED TO APPEAR IN THE COUNSELLING WITH THE CONDITION THAT THEY HAVE TO DEPOSIT ORIGINAL CERTIFICATES WITHIN THE STIPULATED TIME AS DECIDED BY THE ADMISSION COMMITTEE. You will not be admitted in above Course without ORIGINAL admit card AND RANK LETTER of AIPMT-2014 7. For claiming seats in the subsequent rounds of counselling, it is mandatory for the candidates to attend the first round of counselling. 8. SC/ST Candidates shall have to submit a recent Caste Certificate in Original from the competent authority as prescribed in AIPMT-2014 information booklet. 9. OBC Candidates shall have to submit a recent (within 6 month) OBC-non creamy layer Caste Certificate issued by a competent authority as prescribed in AIPMT-2014 information booklet. Non-creamy layer OBC candidate have to submit Income Tax Return Certificate of their parents of preceding three years. They also have to give an undertaking on an affidavit that they belong to non-creamy layer of OBC Category. 10. You are advised to bring approximately Rs. 20,000/- to cover tuition fee, caution money, hostel fee, and other charges. 11. At the time of admission, you shall submit a duly notarized Bond on a Non-judicial Stamp Paper of Rs 100/- solemnly affirming that you will complete the course of studies, to which you are being admitted for full term. The procedure would be carried out at the time of counselling on being admitted. In case you discontinue the studies at any time on or before 15th September, 2014 you shall have to pay the Director, Institute of Medical Sciences, BHU a sum of Rs. 25,000/- in DD (in case of SC/ST candidate it shall be Rs. 5,000). However, if you leave the studies on or after 16th September, 2014 or any time till the completion of above course, you shall have to pay sum of Rs. 3,00,000/- in DD to the Director, Institute of Medical Sciences, BHU (in case of SC/ST candidates it will be Rs. 75,000/-). 12. The classes of MBBS/BDS courses will start from 11/08/2014. You are advised to come prepared accordingly to join the classes. 13. Your admission will be cancelled on any of the following grounds: a. Failure to deposit all prescribed fees by the due date. b. Failure to deposit original certificates within the stipulated time as decided by the admission committee. c. Failure to join the regular classes of the course in time. 14. Your admission shall be cancelled at any stage of the MBBS/BDS course if at any time it is discovered that you have submitted any fake/forged Certificate/ Mark sheets/Photograph or any other relevant document. 15. Your admission shall be subject to the statutes, ordinance, rules and regulations framed from time to time by Banaras Hindu University. 16. The admission process may go on for two days hence, you are requested to come prepared and make your own arrangement for stay at Varanasi. No TA/DA will be provided by the Institute. NOTE: Candidates are advised to read the instructions carefully and come prepared accordingly:
