The Mommy & Me class is an educational program of the Forest Hills Jewish Center. It is here we begin to meet the challenge of making your child intimately rooted in and connected to the Jewish experience in its totality, and thus we accept only Jewish children into the program.

All fees are non-refundable.

The Forest Hills Jewish Center reserves the right to terminate this agreement at its discretion when it deems such action to be necessary. In such an event, tuition will be refunded on a prorated basis.

We, the undersigned, hereby enroll our child in the Forest Hills Jewish Center Mommy & Me Program beginning September 17th, 2013. The undersigned are the parents of this child, who is Jewish (a Jewish child is one whose mother is Jewish). Should you have any questions pertaining to the religious status of your child because of adoption or conversion, we request you discuss this with the Rabbi of this synagogue.
