Compiled by Dr Tokunboh Odubanjo (UNILAG/LUTH) and Dr. Uche Igbokwe The TSL workshops series for November 2012 held between 13th and 27th November in 4 centres located in the various regions of the country viz. Ahmadu Bello Univeristy Teaching Hospital (ABUTH), Zaria; Lagos University Teaching Hospital (LUTH), Lagos; University of Calabar Teaching Hospital (UCTH), Calabar; and University of Benin Teaching Hospital (UBTH), Benin, Nigeria. This workshop series was endorsed by the West-African division of the International Academy of Pathologists (WADIAP) who obtained CME credits from the Nigerian Medical Council. It was also supported by the Pathological Society of Great Britain and Ireland with a travel grant (reference number OS 2012 09 03) for the faculty from the United Kingdom comprising: Dr Olorunda Rotimi, Consultant Gastrointestinal Pathologist, Leeds Teaching Hospitals NHS Trust, United Kingdom Dr Uche Igbokwe, Consultant Pathologist, Queen’s Hospital, Romford, Essex, United Kingdom As obtained in the past, the workshops series was preceded by an External quality assessment (EQA) Circulation (003) with a theme of endoscopic GI pathology. Cases 1-18 were Endoscopic Gastrointestinal pathology cases while 19-25 were various surgical pathology cases selected to illustrate the use of immunohistochemistry in routine diagnosis. Responses received were analysed prior to the workshops, which informed both the participants and the faculty. A total of 30 responses including 11 consultants were received. This represented a significant improvement on circulation 002 and is the best so far. However as before, informal feedback indicates that at least twice the number of pathologists who submitted a response did look at the slides. The total number of pathologists and residents in Nigeria is estimated to be in the region of 150-200. So there is a long way to go yet in persuading the majority to submit a response and maximise the benefits of the EQA scheme. It can be said that the TSL workshops in diagnostic histopathology is evolving into a core centred on facilitated discussions/teaching around the cases of the EQA and additional sessions including guest lectures on other interesting issues in medicine related to pathology. The additional session/lectures in now being determined by the interests of plans of each host centre/region, so that in some cases the EQA meeting session only became part of a much larger meeting. This was the case in Zaria, Calabar and Benin. As a result of this development we are seeing a very small but significant number of people either chose to attend workshops outside their region or attend more than one workshop. The estimated attendance by pathologists of this Nov 2012 workshops series is 100. An estimate rather than an exact number was because in Benin where a multidisciplinary week-long conference was held, the pathologists were not sufficiently differentiated from other participants on the attendance register (I certainly recognised many names from previous workshops who registered without using the title Dr.). Ahmadu Bello University Teaching Hospital, Zaria Kaduna state. The whole of Wednesday 14th November was devoted to discussing the EQA cases with the 25 pathologists in attendance. The discussions were facilitated by Dr Uche Igbokwe with some teaching as necessary. During the rest of the week there was a multidisciplinary program of training in cervical cancer screening as part of an even larger 2 week program of increasing awareness about cervical cancer prevention and research. Attendants included pathologists, nursing staff, O&G doctors and others involved in cervical cancer. This cervical cancer training during the week was also delivered by TSL workshops. Prof. Banjo came with 2 medical laboratory staff (Osiagwu Daniel and Adeteye Olawale) from Medilag and LUTH along with a load of laboratory equipment. There were lectures on the essential elements of a cervical cancer screening program (based on UK experience) by Dr. Igbokwe and on lessons from an important research project on cervical cancer in Lagos by Prof. Banjo. A microscopy training session in gynae cytology for the pathologists was conducted by Dr. Igbokwe. There was also training in HPV detection by PCR/LBC preparation for the Laboratory scientists and training in smear taking for Nurses and O&G resident doctors. An important immediate outcome of this cervical cancer training was the unpacking and installation of some laboratory equipment that had been lying idle for a while at the pathology department of ABUTH. The majority of pathologists attending were from ABUTH, but there were also attendants from Sokoto and Bauchi. Unfortunately unlike the first time a year ago there was no attendance from Kano which is a big department only 3 hrs away. There was some discussion about how to remedy this situation and increase participation of pathologists in the Nothern region of the country.
Lagos University Teaching Hospital, Lagos A total of 24 pathologists attended the workshop in Lagos. Of significance was the presence for the first time of a contigent from Ibadan. This was a small contingent of one consultant and two residents, because a number of others had chosen to attend the workshops at Benin and Calabar. This is significant progress in establishing a South-West regional EQA meeting of substance. There was also an attendant from Enugu. On the first day which was Friday 16th November, there was a review of EQA circulation 003 cases 19-25 facilitated by Dr Igbokwe. This was followed in the afternoon by a keynote lecture on the vital elements of a cervical cancer screening program (based on experience from the UK) delivered by Dr U. Igbokwe to a much larger audience including pathologists, obstetricians and gynaecologists and community health physicians. The next/second day was taken by the guest faculty: Dr Olorunda Rotimi. He facilitated the discussion/review of the endoscopic GI cases (1-18) in the EQA circulation 003 with some teaching as necessary. In addition Dr. Rotimi talked through a video he had prepared to demonstrate the gross dissection of colorectal cancer specimen with a lot of practical tips from his routine practice. University of Calabar Teaching Hospital, Calabar In Calabar, the Nov 2012 TSL workshops formed a large part of a preconference pathology workshop for the 6th African Middle East Association of Gastroenterology (AMAGE) congress hosted by the Society for Gastroenterology and Hepathology in Nigeria (SOGHIN). The preconference workshop spanned two days (20th-21st November, 2012) and was attended by 17 pathologists from Calabar, Ibadan, Lagos, Benin, Ekiti, Jos, Uyo, Nnewi and Abuja. Along with some local participants, Dr Rotimi and Dr. Igbokwe gave lectures on different aspects of gastrointestinal pathology. Dr. Rotimi facilitated the discussions on the GI pathology cases of the EQA (1-18) as well as additional material from his teaching collection. There were also discussions and adoption of minimum datasets for reporting colorectal and oesophageal cancers in Nigeria. University of Benin Teaching Hospital, Benin In Benin as before, the TSL workshop formed an intergral part of the annual departmental conference. It would be correct to say that the Benin annual pathology department conference borne out of hard work and imagination of the local pathologists was inspired a couple of years ago by the TSL workshops when Prof. Banjo was the first guest/key note lecturer. The conference has grown rapidly to occupy a unique position as one that brings together pathologists and laboratory scientists for a week of multidisciplinary learning and interaction. This 3rd annual conference which ran from 26th to 29th Novemer was attended by up to 85 persons (including up to 40 pathologists and a veterinary pathologist) from 16 different institutions all over Nigeria! The guest faculty were Prof. Stefan Dirnhofer – a haematopathologist from Switzerland, Dr Uche Igbokwe of TSL workshops and Mrs ‘Kemi Erhiaganoma – a Biomedical scientist from Canada. All the EQA cases were discussed in sessions alternating with Prof. Dirhhofer’s lectures on 26th and 27th November. An opening ceremony held on the morning of Wednesday 28th was followed by the practical laboratory training sessions on immunohistochemistry extending till the end of the conference on Thursday 29th. The good Prof. was fascinated by the scheme (Switzerland apparently does not have diagnotic EQA schemes) and actively participated in the discussions. It is noteworthy that pathologists from Benin accounted for nearly half of all the EQA responses received – a reflection of the dynamism of the current HOD Dr. D. Obaseki. Without doubt, Benin as a centre is setting the pace for what TSL workshops was set up to achieve – inspire unique efforts to develop pathology practice in all regions of the country that is owned by the local pathologists.
