The Elkridge Youth Organization Soccer Program Registration Agreement By registering yourself or your child you are agreeing to the following statements ... even if you don't read them! So ... it might be wise to take a few minutes and read everything. 1) REFUND POLICY - Please note that there will be a $30 non-refundable administrative fee for any registration cancellations prior to team formation and practices. Once teams are formed or practices begin no refunds will be given. See the Soccer Policy, which can be found on the EYO website at www.eyosports.org -> Soccer, for more details. 2) LATE REGISTRATION - Players registering after Regular Registration closes will pay a late fee and may be placed on waiting lists if leagues are full. Some leagues may close at the end of regular registration if they are at, or above, capacity. 3) EXPECTED BEHAVIOR - Please note that in registering your child for participation in EYO Sports programs you agree to abide by the following code of conduct and the Parents/Players/Coaches code of ethics: Good sportsmanship is viewed by the Elkridge Youth Organization as a commitment to fair play, ethical behavior, and integrity. In perception and practice, sportsmanship is defined as those qualities, which are characterized by generosity and genuine concern for others. Individuals, regardless of their role in activities, are expected to be aware of their influence on the behavior of others and model good sportsmanship. Expected behavior of parents, coaches and spectators includes: respecting decisions made by officials; being an exemplary role model by positively supporting teams; respecting fans, coaches, officials and participants. Unacceptable behavior includes: booing or heckling an official's decision; criticizing officials in any way; displays of temper with an official's call; yelling or making statements that antagonize opponents; blaming loss of game on officials, coaches, or participants and the use of profanity or displays of anger that draw attention from the game. Any discussion between a coach/parent/official/player should be held in a non-threatening manner, or the discussion should be immediately terminated and be continued with the commissioner. The detailed Code of Conduct and the Parents, Players, and Coaches Code of Ethics can be found in the Soccer Policy, which can be found on the EYO website at www.eyosports.org -> Soccer. Failure to read the detailed Code of Conduct and detailed Parents, Players, and Coaches Codes of Ethics found in the soccer policy does not excuse any action that stands as contradictory to the philosophy contained in these documents and will not limit any disciplinary action that may result from those actions. 4) TEAM FORMATION - You also agree that recreational/instructional teams will be formed in such a way as to make them balanced whenever possible. While EYO soccer encourages players to request a coach and specify friends that they would like to have on their team, we do not guarantee that all requests will be honored. League coordinators will endeavor to honor as many requests as possible - but requests will be honored only if doing so will not cause teams to be unfairly strong or weak. When groups of players, who were on a given team, move up to the next older league it is understood that in many cases these younger players will be split into two or more groups and distributed among the older players to maintain team equity. Every effort will be made to keep smaller groups of these players together. It is reasonable to expect that groups of two-to- four players will stay together. It is also reasonable to expect that groups larger than five players will be split. No refunds will be given to players who withdrawal because one or more of their requests were not honored. Players who withdrawal for this reason will not be allowed to participate in the following soccer season. 5) PHOTO RELEASE - By registering your child for this sports program with the Elkridge Youth Organization (EYO), you give permission to EYO to use your child's image on their website and in promotional materials. You may exclude your child by contacting the soccer commissioner and requesting that your child's image be excluded from the website and any materials. 6) LIABILITY RELEASE - In registering your child with the Elkridge Youth Organization (EYO), you, as the responsible registrant, agree that with your child's entry into this program, you along with your child's other parents or legal guardians (as appropriate) shall indemnify and hold harmless the EYO against any potential damages or liabilities which may occur through your and/or your child's involvement or participation in this program. In no event, shall the EYO or any of its volunteers, staff or agents acting on its behalf be liable for any potential consequential, incidental, direct or indirect damages arising out of or in connection with you and/or your child's entry and/or involvement in this program. 7) PLAYER EQUIPMENT - For safety reason all players must wear shinguards to all practices and games. Players without shinguards will not be allowed to participate. No jewelry is allowed to be worn during practices or games. Pierced earrings will not be permitted to be taped over - even if ears are newly pierced. Please plan new ear piercings accordingly as no refunds will be given once teams are formed. Sports goggles or eyeglass straps are also highly recommended for safety reasons. 8) VOLUNTEERS - EYO Soccer welcomes anyone who wishes to volunteer. There are many tasks that need to be accomplished throughout the soccer season. Some, like coaching, are an on-going commitment. Others, like uniform distribution, are short term, one-time commitments. We have a task that can fit just about any schedule and ability from team parent to field maintenance. Please contact the Soccer Commissioner or League Coordinator for more information. All volunteers must undergo a background check, paid for by EYO, prior to being accepted for a position. 9) COMMUNICATION - Please note that the main method of communication throughout our soccer season will be via email. With over 500 players registering for each of our seasons it is impossible for us to contact everyone individually. It is also not possible for us to monitor email addresses to make sure that they are valid or for us to "register" our email addresses with any spam blocking mechanisms that you may have in place. For this reason ... double check that your email address on file is correct - especially if you have changed providers ... add the email addresses of the soccer commissioner ([email protected], [email protected], and [email protected]) and the appropriate league commissioners - which can be found on the main Soccer page of website - to your "known" email addresses. Check your email often - especially prior to practices and games. This will help insure that you do not miss any vital communications. Soccer Registration Form
