EDUCATION SERVICE PROVIDER (ESP) (FORMERLY MANAGEMENT COMPANY)

Pursuant to the Charter School contract, “If a management company or a combination of contracted professionals will be managing the Charter School, the contract(s) between the Charter School and company(ies) shall be submitted to the Sponsor for review prior to the approval of the Charter School’s contract. Any amendments to these contracts shall be submitted to the Sponsor for approval prior to the execution by the Charter School. A copy of all executed contracts must be provided to the Sponsor within the timeframe provided by the charter contract”.

After reading all three selections below, check ONLY 1 of the choices as it applies to the Charter School. A valid agreement, signed and dated, is required for ALL charter schools that have contracted with a ESP.

1. The charter school has contracted with a ESP to provide education management services. The name of the provider is:

2. Since entering into the last contractual agreement with the District, there has been a change in the ESP. The new provider is:

3. Since entering into the last contractual agreement with the District, there has been an amendment to the agreement with our current ESP. (Provide a signed and dated amendment with the ESP)

4. At this time, the charter school has made no changes to its contracted ESP or its contract with said entity. The name of the current provider is:
