You will not be allowed to go back into the locker room to get it once you are outside.

Cardio/ Mile Make Ups Cardio/ mile make ups are by appointment with your teacher.

This is an opportunity to make up a cardio or mile due to an absence or for having a note excusing a student from physical activity on cardio/mile day. It is not an opportunity for you to make-up a cardio due to a lack of participation resulting in a loss of points.

While we encourage physical activity outside school, participation in a run, race, family activity, etc may not be credited toward a missed mile or cardio or other class work.

7.

10 RAINY DAYS Students are expected to dress everyday. When it is raining, the PE staff will determine whether students change for class. Do not assume that all periods will do the same thing. If your class period is dressing and you are not dressed you will be given a non-suit.

Indoor lessons may include, but not limited to, various health topics, or explanation of the rules to games. Students are expected to complete any assigned work, as this is their participation grade for the day.

INJURIES Middle school students will take care of their own minor injuries, for example cuts and scrapes. Report to your teacher. He/she will instruct you to - wash your hands - clean wound with soap, using a gauze pad - put a Band-Aid on cut and throw away pad and papers - wash hands again

If you fall and feel you need assistance getting up, have a class mate go get a teacher. Report any injury to the head or face immediately. This is the only time a student will be sent to office.

Injuries occurring outside of school, should be treated at home.

8. STUDENT RESPONSIBILITIES

Middle school students are responsible for their learning. It is the responsibility of each student to ask their teacher what they have missed when they are absent and to make arrangements to make up any work, a mile run, for example.

It is a student’s responsibility to bring their P.E. uniform everyday. Students will not be permitted to call home to have their parents bring their uniform. You must have your uniform when entering the locker room. You will not be given a pass to your hall locker, to get your uniform.

Your uniform should be washed weekly. Take your uniform home on Fridays and wash it over the weekend and have it ready for Monday. Clothes in the laundry is not an excuse for failing to dress for class. A second set of P.E. clothes in your locker is a good idea.

If you are feeling ill, in the evening or before school begins, ask your parents to write you a note for P.E. If you do not have a note, you will be expected to do the best you can. If you can’t participate in the class activities and you do not have a note, you will lose participation points. Parent notes are only good for the day you need it.

If you are participating in Abbott after school sports program, you will not be allowed to practice/participate if you are excused from physical activity that day or if you fail to participate in class.

Extended doctor’s medical notes do NOT excuse you from written work, tests quizzes etc. You may not participate in afterschool sports when you have a long- term medical. If you are an Abbott team, you may not participate any day you are excused from PE or fail to participate that day.

Keep the locker room clean. It is your responsibility to clean up after yourself. If drop something – pick it up. Put trash in the trash cans. Make sure you have all all your belongings before leaving the locker room.

Defiance and unsatisfactory behavior will not be tolerated when a substitute teacher is present. There will be consequences for student’s poor choices in class.

You will be held financially responsible for misuse or abuse of any P.E. equipment.

9.

12 PARENT RESPONSIBILITES

Medical Notes- Only a parent/ guardian may write a note excusing their son/ daughter from a physical activity in class, for example, push ups because of an injury to the wrist. A parent/ guardian may not excuse a student from dressing. We focus on what the student can do for that class period. The class activities will be modified as needed. In most cases, students are able to perform the warm ups/stretching at the beginning of class.

A note may be written for 3 consecutive school days, one note each day. After three days, California State Education Code requires a doctor’s note. The doctor’s note should include: - the nature of the injury/illness - limitations = what the students can and/or can’t do = modified , limited or total exclusion from physical activity, - and a release date to return to full physical activity. PLEASE NOTE: If the medical note is incomplete, you may be required to have the doctor complete the SMFC School District Physical Education Exemption Approval Form. This form may be obtained from Lauren Rosales in the main office.

Please plan ahead. The P.E. department cannot accept phone calls, faxes, voice or e-mail. Please do not call the school office to leave a message to excuse your student from an activity. If a student feels ill, they will be sent to the office to call home to leave school.

Example of parent note:

Date Dear Physical Education Teacher’s Name

Please excuse my son, John Jones, 6th Grade, from push-ups today. He injured his wrist last evening.

Thank You, Ms. Jones(parent signature required) Daytime contact phone number or email address(must be included)

The note is valid only for the day(s) specified and must state the reason for the excuse, even if the reason is obvious.

Parents can help by encouraging full participation and student responsibility. Please contact your student’s P.E. teacher with any questions or concerns. It is strongly recommended that you register on school loop to get information regarding your child’s progress. 10. P.E. Uniform Prices for 2016-2017
