Numerator: Number of new NHI registrations requiring subsequent linking by MOH per DHB per quarter. It excludes pre-allocated NHI numbers used, newborn NHI registrations and unidentified patient NHI registrations. It is cumulative across the quarter.

Denominator: Total number of NHI records created per DHB per quarter (excluding pre- allocated NHIs, newborns and Unidentified patient registrations). Ratings (based on previous totals of new registrations created and the number of which were duplicate registrations): Group A: Outstanding Less than or equal to 2.00% Achieved Greater than 2.00% and less than or equal to 4.00% Partial achievement Greater than 4.00% and less than or equal to 6.00% Not achieved Greater than 6.00% Group B: Outstanding Less than or equal to 1.00% Achieved Greater than 1.00% and less than or equal to 3.00% Partial achievement Greater than 3.00% and less than or equal to 4.00% Not achieved Greater than 4.00% Group C: Outstanding Less than or equal to 1.50% Achieved Greater than 1.50% and less than or equal to 6.00% Partial achievement Greater than 6.00% and less than or equal to 12.00% Not achieved Greater than 12.00%
