Snoqualmie Elementary School FIELD TRIP NOTIFICATION & PERMISSION FORM

Date : 9-1-16 Dear Parents:

Our class will be going on a field trip on Wednesday, September 14, 2016_to The Meadowbrook Interpretive Center. We will be leaving school at 9:05 am and returning to school by 3:20 pm.

Transportation will be provided as follows

__x_ Bus transportation will be provided for this trip. __x Our destination is near enough to school that we will be walking on this trip. Teacher/parent supervision will be provided. Please fill out and sign the information below so your child will be able to attend our field trip.  The cost for this field trip is $__12__. This covers the cost of the ticket and bus transportation. Chaperone tickets are $__free_____. Please send a check payable to SES ASB or pay online through “Online Payments” using your Family Access code.
