Required (R) Extra Credit (E) Optional 5 R Book Cover with Title A permanent binder is nice but not required. The cover must include the and Book Back book title and your name. You can decorate any way that you like (school appropriate). 5 R Title Page and Titles for The title page goes inside the book and includes the title of the book, your Chapters name, and the year completed. This is your first page, so make it nice. E Dedication or You can dedicate your book to someone using a single sentence (or more). Acknowledgement page You can also acknowledge anyone who helps you put your book together. You can put these on the same or separate pages. 1 bonus point for each. 5 R Table of Contents After you put your book together, make a title page, so the pages can be easily found. You can include page numbers, but you don’t have to put them in.

Must be a “clean copy” before you were born (no grammar mistakes). 1 Bonus point if you include both. Chapter 2 (Include a title page before your entries): Early Years (Points will be deducted for incorrect grammar) 5 R I arrive Must be a “clean copy” (no grammar mistakes) of the story of your birth or the day you were adopted or the day you arrived at the home you live in. 5 R About My Family Must be a “clean copy” (no grammar mistakes) of your story About my family. 5 R Funny Family Story Must be a “clean copy” (no grammar mistakes) of your funny family story. 5 R Happy Childhood Must be a “clean copy” (no grammar mistakes) of your happy childhood memory. 5 R Fun Elementary Must be a “clean copy” (no grammar mistakes) of your best elementary school memory. 5 R Favorite Relative Must be a “clean copy” (no grammar mistakes) of your favorite relative. E Floor Plan Include the drawing of the floor plan of your home. 1 point. 5 R Brag Page Must be a “clean copy” (no grammar mistakes) of your brag page. 2 R Highs and Lows Include your timeline of the highs and lows of your life. 1 point. 5 R Pictures Must include at least 5 autobiographical pictures (scanned or copied is fine). Make sure you label the picture to say who, a date (if you know), and where or what was happening. You can put the pictures throughout your book or make a Picture Gallery page at the end. E Drawings Include a drawing that you have completed (school appropriate please).
4 R Words of Wisdom Place at least two important quotes or words of wisdom you have heard or read. You can use song lyrics or advice or something a family member says. You can put these throughout your book or make a page at the end. 10 R Final Grading Your book will be graded on the following: including everything required and one of each choice item. Your book must be neat, colorful and creative. This is a final project and should show hard work. Extra credit will be given for anything over and above. Remember that grammar counts. If you have been correcting your papers throughout the grading period, this should be easy.
