OBJECTIVE: This practical exercise is designed to evaluate your ability to prepare a Casualty Report.

MATERIALS NEEDED:

1. AR 638-8 2. FM 1-0 3. Blank Casualty Feeder Card (DA Form 1156) 4. Student Handout 5. Pencil(s) 6. Answer sheet

INSTRUCTIONS: a. You will have 50 minutes to complete this Practical Exercise. b. There are 12 required fields on the DA Form 1156 and more information is given on the situation for this Practical Exercise. You must correctly place information in all 12 required fields plus 5 other areas. You must obtain 13 out of 17 entries to receive a passing score.

. SITUATION:

1. Your battalion has been deployed to a combat zone, grid coordinates AB 67824984, Mosul International Airport. The Unit Identification Code (UIC) is WAGLT0. You witnessed a casualty and must complete a Casualty Feeder Report, DA Form 1156 on the incident.

2. On 16 October 2016 at 1540 hours, you and Private First Class Jack Flowers, 979-89-2386, of HQ 94th INF DIV, Ft. Hood, TX, were guarding third country nationals (TCN’s) when enemy forces attacked. Private First Class Flowers was captured and taken. You are the signature of preparer and John L. Lambeau, LTC, IN, Commanding is the approval authority and date prepared.
