NYSAHA Coach/Volunteer Self Screening Instructions

It is the policy of New York State Amateur Hockey that it will not authorize or sanction in its programs that it directly controls, any volunteer or employee who has routine access to children (anyone under the age of majority) who refuses to consent to be screened by New York State Amateur Hockey (NYSAHA) before he/she is allowed to have routine access to children in NYSAHA programs. Further, it is the policy of NYSAHA that it will require its affiliates to adopt this policy as a condition of its affiliation with NYSAHA.

To satisfy NYSAHA’s screening requirements, go to the background screening site administered by our screening partner, Commercial Investigations at :

If you are a US Citizen, click on the United States Volunteer Begin Screening link.

If you are a Canadian Citizen coaching in New York State, click on the Canada Volunteer Begin Screening link. Use the Drop Down Arrow in the Please Select Association box above to identify which association in NYSAHA you are affiliated with. This will assist the association prepare their Association Screening Certification submission.

Then click on the Screen me button. This will take you to the Applicant Info screen.

This is a secure site and any information used in processing your screening is only used for this purpose.

The red asterisk next to the data input box indicates that this is a required field. You will not be able to advance without adding your information in this field. Your email address is needed so that a copy of the report can be mailed to you following your completion of the screening. The format for your date of birth is MM/DD/YYYY. The Officials Certification Number field is required only for NYS registered officials. Click on next to get to the Address data entry screen.

Continue to fill in your address data and click on next to take you the Payment Screen. The cost to self screen is $25.50. Submit information from a major credit card to complete the purchase. Click on the Pay Now button to confirm payment, create the electronic signature and your report will be prepared. The Report Number off your report is the number you submit to your association as proof that you have self screened. They must include it in the Association Screening Certification form they file with NYSAHA.

That completes your part in the self screening process.
