Letter of engagement

Field of employment/place of work: As of “ENTER DATE”you will be engaged as first reserve physician in the Central Denmark Region and will serve in this position until further notice at“ENTER PLACE OF EMPLOYMENT”. This employment is temporary and shall expire without further notice on “ENTER DATE”.

Working hours: Until further notice, your average working hours per week are agreed at “ENTER NUMBER OF HOURS”. The working hours will be structured in accordance with the collective agreement. Additional work, overtime, staggered working hours, etc. shall take place in accord- ance with the relevant provisions in the collective agreement.

Collective agreement: The employment is subject to the conditions enshrined in the collective agreement between Danske Regioner (Danish Regions) and Yngre Læger (Junior Physicians) for assistant physicians.

Salary: Remuneration for this position shall be an annual base salary of DKK 353,200 (31/03/2000 level). Såfremt der er tale om nedsat tid indsættes: The base amount reflects the annual salary for an employment comprising 37 hours/week, and will be reduced to reflect your average weekly work- ing hours. Centrale tillæg (Der kan kun ydes ét af følgende 3 tillæg ad gangen): A centrally agreed pensionable supplement at an annual sum of DKK 30,400 (31/03/2000 level) will be provided for the National Board of Health's permission to practice independently. A centrally agreed pensionable supplement at an annual sum of DKK 45,500 (31/03/2000 level) will be provided after one year of specialised training has been completed. A centrally agreed pensionable supplement at an annual sum of DKK 57,100 (31/03/2000 level) will be provided after the National Board of Health has approved your status as a specialist. Your pay seniority is determined with reference to the documentation submitted regarding previ- ous employment and will appear on your pay slip. A proposal has been forwarded to the negotiating organisation regarding the provision of a func- tion/qualification salary of DKK AMOUNT annually (31/03/2000 level). Once negotiations are com- plete, you will receive a copy of the agreement form with the negotiated result.

Salary is paid: Salary is paid monthly in arrears. You can access your pay slip through your di- gital mailbox at www.Borger.dk.

Pension scheme: A pension scheme is established in accordance with the collective agreement.

Holiday: You are covered by the “Holiday agreement for staff employed in the Regions”, with the exception of Chapter 8 of the agreement regarding holiday pay and special holiday compensation. In place of Chapter 8 of the holiday agreement, assistant physicians shall be subject to the spe- cial holiday provisions set out in Annex 3 to the holiday agreement. While on holiday, an hourly salary deduction will be made calculated as 1/1924 of gross annual salary (base salary, function salary and qualification salary) per hour, including the employee's own pension contribution.

Illness, etc.: Salary will be paid during illness in accordance with the provisions of the collective agreement. You are covered by the “Agreement on absence for family reasons”.

Termination: The Salaried Employees Act's termination provisions shall apply. However, the em- ployment may be terminated by the physician no sooner than on the first day of a given month with one month’s notice, and may be terminated by the Region with three months’ notice. A pro- bationary engagement with an abbreviated notice period is not possible.

Confidentiality: You are subject to the confidentiality provisions laid down in the Criminal Code. The confidentiality obligation does not cease to apply on leaving. Remuneration on issuance of certificates proceeds in accordance with the collective agreement's provisions in this regard.
