1) Corporate relationship among Applicant, Proposed Grantee, Related Member(s), top tier parent (if other than Applicant), and any affiliates in North Carolina; 2) Physical location of all activities of the entities identified in 1), including location and entity that will conduct the Project 3) Description of activity conducted at each entity identified in 1) 4) If entities in 1) are structured in multiple divisions, identification of divisions, their activities, and reporting structure in those divisions 5) If a private company, names of owners and percent ownership held
