Context The Red Cross is the largest voluntary humanitarian organisation in the world. It promotes humanitarian values and provides emergency help to people in short term crisis in the UK and overseas. Within the UK, the British Red Cross delivers through it’s 50,000 volunteers and 3,000 staff.

Scope The Service Co-ordinator function provides coordination & support for staff and volunteers delivering CiTH modules across each locality. They hold day-to-day responsibility for the supervision of a range of British Red Cross services, collectively termed CiTH.

In Poole hospital the services provided are specifically around Assisted Discharge, facilitating the transition to home from the Rapid Access Consultant Evaluation (RACE) unit, the Emergency department (ED) and other wards for patients in need of support to help them settle in back at home.

Overall Purpose of the Post To co-ordinate a team of staff and volunteers taking referrals from designated Poole hospital staff, assist with creating simple short-term support plans for each discharged patient, co-ordinate and schedule staff/volunteers to transport patients home and settle them in within 2 hours of receiving the referral and to plan and allocate subsequent telephone calls or home visits for up to 3 times a day in the 72 hour period post hospital discharge.

Main responsibilities and duties 1. To be a member of the RACE unit’s multidisciplinary discharge team (MDT) in order to be aware of those patients becoming ready and able to go home. 2. To be responsible for the day-to-day delivery of the Poole Assisted Discharge service in line with service standards, recognised good practice and contractual requirements.

3. Where necessary, assist or deputise for other team members or volunteers in the delivery of the service.

4. To assign patients to an available Discharge Support Worker (DSW) or volunteer and facilitate transport home and settle the patient in as and when required.

5. To ensure each patient referred is either visited or telephoned up to 3 times daily for the first 72 hours post discharge.

6. To manage and monitor all referrals in a timely and efficient manner.

7. To maintain appropriate records and capture relevant statistical and qualitative data for internal and external use.

8. To ensure the service is patient centred and that their views inform and shape the continued development of the service.

9. To ensure that all team members maintain effective records and that all data is managed appropriately and in accordance with legislation, good practice and British Red Cross policies and procedures.

10. To take a full and active role in the recruitment, induction, supervision and development of staff and volunteers to the team in accordance with British Red Cross policies and procedures.

11. To comply with all health and safety legislation, standards and policies.

12. To provide weekly, monthly, quarterly and annual service reports and statistics as required.

13. To achieve personal and professional objectives as agreed with their line manager.

14. To support and assist in the provision of 24 hour emergency cover as required.

16. To undertake other service relevant duties as requested by the line manager.

EDUCATION Educated to A level standard or equivalent experience
