Equipment Checkout Policy and Approval Form

POD equipment is primarily for training use. However, equipment may be checked out on a limited basis for a two week maximum, through special arrangement providing dates/times do not conflict with scheduled POD trainings.

Check out approvals will be based upon the following criteria: 1. The number of requests and availability of equipment (first come, first served basis) 2. The sensitivity of equipment and potential equipment damage 3. The need for accountability and protection of NKU property

Following are the policies adopted by POD for equipment checkout:  NKU faculty and staff may check out equipment as a supplement to training conducted by POD or to “test” equipment to see if they would like to have their department purchase said equipment for them.  Equipment checkout should be professional development or work related.  It is the responsibility of faculty or staff to ensure that the borrowed equipment is returned in the same checkout condition. Retribution for damaged or stolen equipment will be the responsibility of the faculty or staff member checking out the equipment.  Equipment is not to be checked out for student use or to be checked out by faculty to have their students using it. The only exception to this is the clickers.

Checkout/Check-in  All equipment will be thoroughly tested at both “checkout” and “check-in” to ensure that it is working properly.  Equipment cannot be checked out for extended periods of time without the prior consent of the POD manager.

The borrower of this equipment shall use the equipment lawfully, in a careful and proper manner, and shall not alter the equipment. The borrower shall bear any and all risks of loss and damage to the equipment.
