DRUG AND ALCOHOL QUARTERLY MONITORING REPORT

1. Purpose of report
To provide the Joint Commissioning Partnership Board (JCPB) with the latest monitoring information in relation to drug and alcohol services.
2. Summary
This report aims to update the board with the latest performance reported by the National Treatment Agency, current performance indicators drawn from local data, and a brief explanation of changes to national targets to be implemented from April 2008 for the next 3 year planning cycle. It will include a summary of any additional issues raised at the most recent Joint Commissioning Group and includes a summary of issues relating to alcohol.
3. Performance targets
3.1 Overview
From 1st April 2008 to 31st March 2011 performance targets will be based on growth of Heroin and Crack users, PDU’s (Problematic Drug Users) in effective treatment (Those clients in treatment for 12 weeks or more or discharged in a care planned way before 12 weeks.) The two measures being NI40 and Vital signs. Hertfordshire’s target for both measures for 2009 is 1% growth on the 2008 baseline; this will then change in 2009/10 which will be reported nearer the time.
NI 40: a growth of 1% in year 1 (HCC Target) Vital signs: a growth of 1% per annum for all 3 years.(PCT Target) Both targets will be based on a baseline of 1636

Alcohol Treatment
There are no national targets or KPIs set for alcohol, however we are now able to report numbers in treatment as recorded by the National Drug Treatment Service, and will use them to inform commissioning plans.

3.3 Criminal justice interventions
Drug Intervention Programme (DIP)
Will no longer be measured as a separate entity, but will contribute to the overall treatment targets. It is important to note that DIP clients are predominately Heroin or Crack users so will have a significant impact of the overall PDU target:
Court Orders
For 2008-09 the targets of Drug Rehabilitation Requirements are:
180 starts with 75% offenders retained in treatment for 12 weeks.
Current performance to end June 2008 97 starts
For 2008-09 the targets of Alcohol Treatment Requirements are:
300 starts with 175 satisfactory completions (no new targets for 2008/9).
Current Performance to end Jun 2008 89 starts
3.4 Providers
Turning Point (HertsReach)
As a result of the new emphasis being placed on Crack misusers Turning Point/ Hertsreach are running a Crack specific service from their Hemel base, we would hope this will help the retention of this client group in effective treatment, and if successful could be provided across all Hertsreach services. There are already plans to provide this service as part of the DIP and this is due to start by the end of 2008.

As with Hertsreach, WDP are planning to introduce a Crack specific program from their Hertford and Stevenage sites. Both services are planned for introduction in October and will add to our ability to meet the need identified in our needs assessment 2007/8 and our treatment plan for 2008/9. As a result of successful bids by the national office, WDP Hertfordshire has been chosen to run 2 new projects from their Hertfordshire service. Under their partner organisations name, Core Kids they will provide a menu of activities from their Stevenage base designed to meet the needs of families affected by Drug or Alcohol use. This is due to commence in October. An additional award from Comic Relief will support a worker for a 3 year Alcohol Hidden Harm project again focussing on families. Both contracts will have a dedicated member of staff but allow for additional sessional staff to deliver family therapy and possibly interventions with children.
Hertfordshire Partnership Foundation Trust (HPFT)
The transfer of data to carenotes has now been completed. There were a number of issues highlighted by National Drug Treatment Monitoring System following the migration from their old systems and this caused some concerns as to the impact of the data on the baseline target set by the NTA this year, however after some lengthy and time consuming analysis the final agreed figure was set at the figure reported above We are in discussions with HPFT regarding the ongoing management of the supervised consumption budget. This is currently held by JCT but we feel it would make better sense for HPFT to manage the budget at an agreed level as they have direct responsibly for the amount of supervision delivered. It is hoped this will achieve a balanced budget, as has been successfully achieved by the delegation of the Rehab budget to them.
3.5. JCT/Commissioning Plans 2007/08
The Practice Based Commissioning Investments highlighted in the previous report are all going ahead; these initiatives for Stevenage will run alongside and add to the PCT growth initiatives planned across the county.
3.6 Alcohol Services
Alcohol growth
The 4 initiatives proposed and agreed by the PCT are currently being progressed, these are: Reduction of the Alcohol Detox waiting list: Now being actioned, with additional beds agreed with our current providers.
Hospital Brief interventions: Posts have be advertised by Turning Point and they hope to have appointments in place for early November
Alcohol Counselling: 2 bids from local providers have been received. These will be assessed and services can be started almost immediately upon appointment.
0e27c22ebc8af792f414d2f5a9a56750.doc 3 Brief intervention primary care training: Alcohol Concern will be providing this with the first group of sessions planned for early December, followed by a wide range of dates and times in the new year.
3.7 Finance plans 2008/9 - 2010/11
No alteration to budget is noted this quarter
3.8 HCC/NTA performance measures
As part of the ongoing NTA/HCC annual audits, Hertfordshire this year scored Good for Tier 4 and Diversity.
