THIS CAUSE having come before the Court on the Motion of Counsel for

______to Withdraw from further Representation and appropriate notice having been given to all parties of the hearing before the Court, it is hereby

ORDERED AND ADJUDGED as follows:

1. The motion is GRANTED and ______, is permitted to withdraw as attorney for ______, forthwith.

Counsel shall have no further obligation in this matter.

2. Within twenty (20) days of the signing of this Order Plaintiff/Defendant shall either:

A) Retain a new attorney and have the attorney file a written appearance; or

B) File a written notification with the clerk advising the Court that the party

wishes to represent himself or herself. (Note: This option is not available to

corporations. Corporations may only appear through counsel. See Nicholson

- 1- Supply Co. v. First Federal Savings & Loans Assoc. of Hardee Cty., 184 So.

2d 438 (Fla. 2d DCA 1966).

3. Until new counsel appears, Plaintiff/Defendant may be noticed at the address reflected at the bottom of the second page of this Order.

4. Failure of Plaintiff/Defendant to comply with the provisions of A) or B) above will create a presumption that the said party no longer desires to have its position represented in the law suit and sanctions may be imposed by the Court, either upon the

Court’s own motion or the motion of opposing counsel. Such sanction may include the striking of Plaintiff’s/Defendant’s pleadings in the above styled lawsuit.
