Chicago Bandits 2011 National Anthem Volunteer Questionnaire

Volunteers interested in performing the National Anthem at a Chicago Bandits game are invited to complete and return a copy of this form to the Bandits office to be considered for a date during the 2011 season. Please include a non-returnable DVD or CD and a website or YouTube URL if applicable. We will contact selected volunteers to schedule available dates on a space available basis. Once the schedule is filled, remaining volunteers will be kept on a waiting list for potential future openings.

Please note we typically have more volunteers than we have dates to offer. Therefore, a maximum of 30 performers will receive one free ticket for the game. All performers and guests over the allotted 30 will need to purchase a ticket for the game. We cannot issue ‘passes’ for performers or guests who are not staying for the game. Finally, we ask that the contact person for the game date please sign this form before returning it, as acknowledgement that you have read and understand these procedures.
