If you can’t submit the nomination via e-mail, complete this form and submit it by mail, postmarked by Dec. 1. Nominations sent by mail must include a total of eight copies of this completed form, eight copies of the resume, and eight copies of each recommendation. For either method of submission, do not attach published articles.

Identify the value of the research and describe its recognized significant to the profession, either in terms of application of industrial engineering theory or practices or in terms of the development of new or extended industrial engineering theory or practice. Describe the method of accomplishment. Identify the nature of the research undertaken, its scope, the source of research funding, and the nominee’s role in furthering the research (i.e., principal investigator, consultant, doctoral candidate, or supervisor).
Describe the specific value of the accomplishment. What are the benefits of the research? Specific practical applications should be included, along with a discussion about how the research or its results have been exploited by industry.
List previous recognition of the research, including professional and civic recognition.
Additional required materials
Three letters of recommendation should be limited to a maximum of two pages each.
The nomination form, the nominator's letter and CV package should be limited to 12 pages.
The entire nomination packet should be no more than 18 pages total
