Today’s Date: Your Name: Please note: while there is nothing stopping you from submitting this form to the Honor Board without your name on it, please be advised that without knowing whom to contact about this case, the Honor Board’s possible actions are limited.

If you would like your name to be kept anonymous, please state your reasons why. As a general rule during investigations and hearings, the Honor Board will allow the student who has been reported to the Honor Board to know the identity of the reporter and/or aggrieved parties. However, the Honor Board understands that there may be situations (such as sexual assault and rape reports) where it is not appropriate for the reporter or aggrieved parties to be named to the accused student. In these cases the leadership team of the Honor Board (Chair, Vice Chair, and Adviser) may grant anonymity to the reporter or aggrieved parties. The leadership team of the Honor Board may also choose to seek the advice of counsel, Olin’s Title IX adviser and/or the Dean of Student Life in making a decision about anonymity.

List the clauses of the Honor Code or related policies that you believe have been violated by this incident and why. The investigative team will discuss these charges with you.

List the names of any other people with substantive knowledge of the alleged violation. Where possible, please note next to each name a brief description of that person's connection to or knowledge of the event(s) described above.

Note: A reporter has the choice to file a report criminal in nature (violates a state or federal law) to the Honor Board, Public Safety, or both. For more information, see Section 2 of the Honor Code, "College Jurisdiction,” and Appendix F: Privacy Policies in the Student Handbook.
