Q1: Why am I having trouble logging into the portal? A: If you have submitted your Statement of Intent to Register (SIR/SLR), please allow 2 business days before attempting to log into the Ashe Secure Portal.

Q2: How do I upload my immunization records? A: You must log into the secure patient portal and complete the form labeled “Immunization Compliance / Health Clearance”. From that page you can upload/scan your supporting documents.

Q3: I messed up inputting my immunization records, how do I fix that? A: Please email us at [email protected] with the subject line “reset immunization form,” your UID and full name, and we will reset your form within 2 business days

Q4: When is the deadline? A: Sept 19, 2016

Q5: Will I be penalized if I do not complete requirements prior to deadline A: We may put a hold on your registration for the winter quarter.

Q6: What is considered a prolonged period in a foreign country before I have to be tested for TB? A: While it would be welcomed, no evidence-based data exists that identifies the amount of time spent in a given high-risk country that constitutes significant exposure. * The significance of the travel exposure should be discussed with a health care provider and evaluated.

Q7: I do not have a doctor; can I just send you my immunization records and be cleared? A: You must log into the secure patient portal and complete the forms labeled “Immunization Compliance / Health Clearance” & “TB Screening”. Please upload/scan your supporting documents for verification.

Q8: Can I request a personal or religious belief exemption? A: Only Medical Exemptions are allowed. Please see our Immunization Policy FAQ for a more detailed explanation.

Q9: Do I get a confirmation email once I fulfil the requirement? A: No. However you can check the Immunization tab in the secure patient portal to see if you have successfully entered your information correctly. Q10: I entered all my information but it still says “unverified” A: Your Immunizations will say “unverified” unless an Ashe staff member has reviewed them. Please do not contact the clinic to have your records verified. This function will be performed on a batch basis quarterly.

Q11: Where can I mail in my immunization record? A: Please do not mail any documents. Submit everything via The Secure Portal

Q12: I am an incoming freshman do I still need to comply with the Hepatitis B requirement? A: Hepatitis B is no longer required for incoming freshman. Please check the Immunization Requirement page for all required and recommended vaccines.

Q13: If I was an undergraduate at UCLA and now entering as a graduate school, do I need to comply with the immunization requirements? A: Yes, ALL incoming students will need to comply.
