Ankeny Centennial High School/Northview Middle School Parent Permission Slip Girls’ State Volleyball Tournament 2015 US Cellular Center, Cedar Rapids, Iowa Tickets $10.00

For any High School student (9-12) to be dismissed from school to attend the Girls’ State Volleyball Tournament the following guidelines must be followed:

1. Tickets must be purchased from the Activities Office and are to be purchased between 7:30am and 3:45pm prior to the day of the event. Tickets WILL NOT be available at school for the Friday Finals game. In order to purchase a ticket, a signed parent permission slip must be presented to the ticket seller along with the payment.

2. Students standing in line to purchase tickets before school must still report to their 1st hour class. No tardies will be excused to any class period for the purpose of purchasing tickets.

3. Tuesday, November 10th – Students will be dismissed from school during 3rd period at 10:15am (Permission slip must be returned by 4:00pm on Monday, November 9th)

4. Thursday, November 12th and Friday November 13th – Students will be dismissed from school all day on the day of the event. Permission slip must be returned by 4:00 pm prior to the day of the game. Volleyball games begin at 10:00am.

5. Any student who does not follow the prescribed procedure will be marked as unexcused.

I understand that in order for them to attend they must adhere to the guidelines listed above.
