Position Title: Independent Living Information and Referral Specialist

Employment Classification: Full-time Regular, non-exempt

Work Site: Main Office usually

General Statement of Duties Provide Agency Information and Referral Services. Update referral resources, both web based and network based, for providing independent living services, especially Information and Referral services. Coordinate information and referral services systems. Assist with training of volunteers in information and referral. Provide independent living individual and community services as assigned. Demonstrate professional conduct, and sustain technical proficiency in all work areas. Promote and preserve Independent Living philosophy and consumer control.

General Statement of Duties and Responsibilities as assigned by Supervisor:

1. Coordinate Agency systems for Information and Referral services to consumer, family members, service providers, and community at large. 2. Provide Information and Referral services for Agency. 3. Insure staff awareness of Information and Referral Resources. 4. Collaborate with Data System staff to insure that all Referral Resources are organized electronically and accessibly for use by Agency staff, consumers and community at large. 5. Organize and keep up to date all paper Resources in Reception Area. 6. Mentor assigned volunteers on provision of Information and Referral services and resources. 7. Network with local service providers and community organizations to obtain information on their services for Agency resources and to share information with them on ILCNSCA programs, services and IL philosophy. 8. Represent Agency with ADRC partners Information and Referral programs for insuring referrals from and to ADRC partners. 9. Assist with Information & Referral Follow-up survey working with Data Specialist. 10.Share local resources with MADIL and insure that all staff of Agency use MADIL for information and referral resources. 11.Maintain Agency web page links to resources and insure Yellow pages information is up to date, as well as work with Data Systems staff on annual production of Yellow 097fe8d344dbb22180dd4826b43b21e4.doc pages distribution. 12. As per contracts and Agency plan provide core services as follows to consumer caseload:  Provide Information & Referral services;  Provide intake and assessment of eligibility;  Provide peer counseling and skills training services;  Provide individual and community advocacy services. 13.Maintain accurate Consumer Service Records (CSR) following all ILCNSCA policies, procedures, and standards. 14.Develop and periodically review an Independent Living Service Plan with each consumer who has a current CSR. 15.Accurate CSR data collection and daily data entry. 16.Complete all required data collection in IDMS. 17.Meet contract deliverables. 18.Prepare monthly, quarterly, and annual reports as required. 19.Perform other tasks such as Independent Times Constant Contact newsletter, as requested by supervisor or Executive Director. 20.Seek supervision and training as needed.

Qualifications: 1. Bachelor Degree in Human Services, a related field, or equivalent life experience. 2. Person with disability and/or with skills necessary to establish effective peer relationships. 3. Excellent interpersonal, verbal and written communication skills in English. 4. Computer literate in all Microsoft Office applications, including MS Word, Access database management, Excel, and Outlook. 5. Team player with agency staff as well as with other stakeholders on advocacy issues. 6. Knowledge of Federal, State and local agencies serving people with disabilities. 7. Knowledge of service delivery systems. 8. Commitment and skills to work as part of ILCNSCA team. 9. Experience maintaining consumer and agency confidentiality. 10. Responsible for own transportation. 11. Bi-lingual English/Spanish desired. 12. Must be committed to Independent Living Philosophy and the IL paradigm.

Salary: $13.00 per hour to start.

Hours: 35 hours per week.

Benefits: As stated in agency Employee Manual.

Review Policy: Supervisor will complete performance appraisal with employee at the completion of a three-month probation period. Job duties and assignments will be 097fe8d344dbb22180dd4826b43b21e4.doc reviewed at this time. Thereafter, performance appraisals will be completed and reviewed annually.
