Students who are planning on studying away from Smith College during either semester of their senior year should complete this form in its entirety. Incomplete forms will not be considered.

You need to:

Meet with and obtain the signatures of your Adviser and Department Chair prior to submission.

Complete a separate form for each of your majors if you have two majors.

Resolve any credit shortages prior to studying away your senior year.

Submit an Off Campus Study/Personal Leave Form with your Senior Year Elsewhere form.

Read and abide by the Guidelines for Transfer Credit.

Submit course descriptions for courses you intend to take elsewhere.

File a Certification of the Major Form when you receive notification from the Registrar.

Once all forms are in order, your plan will be reviewed by the Class Deans. Written approval will be sent to your campus box.

If for any reason there is a change in your plan you must notify your Class Dean. Any change in major courses must be approved by your major adviser.
