Incomplete forms will not be considered.</p><p>You need to:</p><p>Meet with and obtain the signatures of your Adviser and Department Chair prior to submission.</p><p>Complete a separate form for each of your majors if you have two majors.</p><p>Resolve any credit shortages prior to studying away your senior year.</p><p>Submit an Off Campus Study/Personal Leave Form with your Senior Year Elsewhere form. </p><p>Read and abide by the Guidelines for Transfer Credit.</p><p>Submit course descriptions for courses you intend to take elsewhere.</p><p>File a Certification of the Major Form when you receive notification from the Registrar.</p><p>Once all forms are in order, your plan will be reviewed by the Class Deans. Written approval will be sent to your campus box. </p><p>If for any reason there is a change in your plan you must notify your Class Dean.
