Neptune Township School District - Neptune, New Jersey www.neptuneschools.org Preschool 3 & 4 Year Old Enrollment 2016-2017 School Year

Registration for all Neptune Township three (3) and four (4) year old children eligible to enter preschool in September, 2016 will open on March 15, 2016 through the District’s EZ School Online Registration Portal (directions are noted on the District website). Finalization of the preschool registration will end with an appointment at the Registrar’s Office. Please note: Only natural/adoptive parents or legal guardians can register their child. New preschoolers will be placed in their home school based on space availability. A lottery will be used in the event maximum enrollment is reached at the home school. Overflow enrollment will attend the Early Childhood Center. If needed, the LOTTERY NIGHT for all schools will be June 9, 2016 beginning at 6pm at ECC, 11 Memorial Drive. Who Is Eligible for this Free Program? Only Neptune Township children who are three (3) years old or four (4) years old on or before October 1, 2016. Child does not have to be fully toilet trained in order to start program. STUDENTS ALREADY ENROLLED IN THE PROGRAM DO NOT HAVE TO REREGISTER.

Is There A Registration Packet? No. All registration is done online through the District website at www.neptuneschools.org (directions are included), and finalized with an appointment scheduled through the Registrar’s Office, Midtown Community School, 732-776-2200, x3632.

What Must I Do On Registration Day?  Parents/Guardians with computer access can begin the online registration process at home beginning March 15, 2016. All original documentation noted below must be brought to the final appointment at the Registrar’s Office.  Parents/Guardians without computer access can begin the online registration process at the Registrar’s Office. All original documentation noted bellow must be brought to the final appointment at the Registrar’s Office.  After the information has been entered online, an appointment must be made with the Registrar’s Office by calling 732- 776-2200, x3632 between hours of 8am and 3pm. All documentation must be brought to the appointment in order to complete the process. Students are not enrolled until documents are accepted and the appointment has been completed.

Registration Documentation needed for this Free Program  Original birth certificate (or custody paperwork, if applicable). NOTE: Full name of natural/adoptive parents on birth certificate or court appointed guardian must also be on the proof of residency requirements.  Proof of residency in Neptune Township--need 4 ORIGINAL items with 1 being a utility bill (on website Guidelines). If you are living with a Neptune resident, an affidavit will have to be completed and notarized, and two proofs of residency from the homeowner or lease must be brought to appointment.  Proof of physical exam (within past 12 months) or proof of appointment: PRINT OFF WEBSITE FOR DOCTOR  Proof of Immunizations: o DPT-series of 4, Polio-series of 3 o MMR, Varicella, HIB, PCV (Pneumococcal)—1 dose after first birthday o ANNUAL Influenza Vaccine to be administered between September and December 2016 Flu shots given before September 1, 2016 will not be accepted for 2016-2017.
