A complete answer to the following questions will enable us to be more familiar with your request and requirements, and will help us find the rescue dog to match your needs and expectations.

Would you consider fostering a Special-Needs Rescue Animal such as one who requires medication for a permanent but controlled condition?

If yes, please give their ages (Please note that we try to be as careful as we can when placing dogs in homes, especially when children are present, but we often have limited information regarding a rescue animals history or temperament).

I/We attest that the Terms and Conditions of Fostering as stated below have been read in full by me/us and I/we understand that is part of the fostering process and will be enforced. I/we attest that the information provided on this application is true and accurate to the best of my/our knowledge. I/we attest that we have retained a copy of the Terms and Conditions of Adoption and also understand that completion and submission of this application is an application for fostering of an animal and does not guarantee adoption of a rescue animal. I/We understand that any misrepresentation of fact may result in the removal of the fostered dog from my/our home. If this application was submitted via email, formal signature(s) will be obtained when the first foster animal is placed in the foster home and will be maintained on file by SBRET. I/We understand that in the event we wish to adopt any SBRET rescue animal I/we will be required to follow the same adoption process as required by all individuals requesting to adopt an animal from SBRET.
