The IRS recently updated some forms that are used at the UW. All current IRS forms can be found at the IRS web page at IRS Forms and Publications.

Form W-8BEN completed by foreign nationals (Nonresident Aliens (NRA)) has been broken up into two forms, W-8BEN for foreign individuals, and W-8BEN-E for use by foreign entities. Notes on Form W-9: Prior Form W-9 had a check box for "Exempt payees" in the top section of the form. The updated form asks for an "Exempt payee code" (if any). Generally, individuals (including sole proprietors) are not exempt from backup withholding. Corporations are exempt from backup withholding for certain payments, such as interest and dividends. If they claim they are exempt from backup withholding, they should complete the form W-9 and mark the exemption code (listed on page 3) to avoid possible erroneous backup withholding, for example, code 1 for an organization exempt under section 501(a). Notes on Form W-8BEN: Prior form W-8BEN was completed by both foreign national individuals and entities. For the first time in 2014, the form W-8BEN has been split into two forms. The new 2014 form W-8BEN is for use solely by individuals. A new form W-8BEN-E is for use by entities. Implementation Timeframe: Please start using the new forms effective immediately. If a payment request has already been submitted to your Dean’s Office you will not be required to update the forms associated with those payments but all future submissions will require the updated forms. As future changes in forms occur Accounting Services will notify you and post the notice on the Tax web page so everyone is kept current.
