The Omega Mu chapter of the Order of Omega strives to produce the highest standards of quality within our Greek community through leadership, philanthropy, education, mutual cooperation, and understanding among our organizations. Our noble goal is to forge the fraternities and sororities of Rensselaer Polytechnic Institute into a national model of excellence in Greek affairs.

Answer the following questions on a separate sheet and attach to this form. Only use sentences when instructed to do so.

1. List all chapter, IFC/Panhel, or Greek life-related offices you have held as well as the average hours per week you dedicated towards each position. For each office, include a one sentence description of its responsibilities.

2. List your involvement in on-campus activities, including the average hours per week you dedicated towards each position. Cite membership in any clubs or service organizations, as well as any non-Greek offices you have held within those organizations (i.e. student government, intramural sports, etc.). For each office held, include a one sentence description of its responsibilities.

3. List your involvement in off-campus activities and/or jobs you have held. Do not include co- ops, internships, or summer positions unless you were actively taking classes from Rensselaer at the same time. Include the average hours per week you dedicated towards each position.

4. List any awards and/or honors you have received from RPI or your Greek organization.

5. Please write a brief description about the attributes you feel you would contribute to the Order of Omega.

6. Please include your resume to provide further information for review.

I hereby certify that all information I have provided within this application is truthful and accurate, to the best of my knowledge. I understand that providing false information will reflect badly upon my organization and may jeopardize future leaders of my organization being admitted to the Order of Omega. If selected for membership in the Order of Omega, I agree to pay a $50.00 initiation fee to the Dean of Students Office. (Please make checks payable to Order of Omega).
