IMMUNIZATIONS

Indiana law requires that students in all grades are required to meet the minimum immunization requirements. The immunization record must include the student’s name and date of birth, the vaccine given and date (month/day/year) of each immunization. Below lists the Indiana school requirements of vaccines with number of doses students must have before attending class.

*One (1) dose of a meningococcal conjugate vaccine (MCV4) vaccine is required and one (1) one booster dose of MCV4 is recommended for grades 11 & 12. Both doses will be required for the 2014-2015 school year.

**Two (2) doses of Hepatitis A vaccine are recommended for all students entering Kindergarten. Both doses will be required for the 2014-2015 school year.

***All students in grades K-12 are required to have two (2) appropriately administered doses of varicella vaccine, or evidence of immunity. For children entering preschool through 5th grade, a signed statement by a health care provider, documenting a diagnosis of varicella or verifying the history of disease, including date (MM/YY) is required.
