<p> IMMUNIZATIONS</p><p>Indiana law requires that students in all grades are required to meet the minimum immunization requirements. The immunization record must include the student’s name and date of birth, the vaccine given and date (month/day/year) of each immunization. Both doses will be required for the 2014-2015 school year.</p><p>**Two (2) doses of Hepatitis A vaccine are recommended for all students entering Kindergarten. Both doses will be required for the 2014-2015 school year.</p><p>***All students in grades K-12 are required to have two (2) appropriately administered doses of varicella vaccine, or evidence of immunity. For children entering preschool through 5th grade, a signed statement by a health care provider, documenting a diagnosis of varicella or verifying the history of disease, including date (MM/YY) is required.</p>
