______(Authorized Signature) (Date) PARADE ROUTE INFORMATION: DECEMBER 3, 2016 IMPORTANT INFORMATION - Line up will be on a first come first in line basis. No line-up position will be assigned. When you arrive at the MIDDLE SCHOOL PARKING LOT, someone will direct you to the line. Those with more than one vehicle participating, should organize a place to meet other than the Middle School parking lot. You should arrive together and be ready to get in line. Vehicles will not be allowed to pass other participants to get next to another float. Riders on floats will have to walk down to where float is located. No vehicles other than those participating in the parade will be allowed to drive past the first parking area at the Middle School parking lot. ENTRANTS SHOULD ENTER THE MIDDLE SCHOOL PARKING LOT and continue down to ABC Drive to line up between 5 p.m.- 6 p.m.. The parade will start at 7 p.m. leaving ABC Drive and turning left onto Hwy 115 then left around the old Court House on Hwy 129 North and end back at the 9th Grade Academy Parking lot. Float judging will begin at 6 p.m. Late participants will not be considered. PARADE POLICY AND REGULATION: Participants in the Christmas in the Mountains Parade must agree to the following rules and regulations. Dimension of vehicle & trailer cannot exceed 45 ft long, 10 ft wide and 13 1/2 ft tall. All mechanical equipment must be in sound, working condition. Motor vehicle operators must have a valid driver’s license and insurance. (Please provide copies) Entrants must be in position by 6 p.m. on the day of the parade. Placement of floats within the line-up is at sole discretion of the Parade Organizers. Entry fees are due with your application and are non-refundable. If you plan to have “walkers” to distribute things to spectators, it must be approved by the Parade Organizers when you turn in your application. All walking entries must be lighted. All floats should be decorated with lights. A minimum of 5,000 lights is recommended for a visible and well-lit entry. Equestrian and animal units must provide their own accompanying clean-up personnel and equipment. Return applications to the White County Chamber of Commerce at 122 North Main Street, Cleveland, GA 30528, by Monday, November 28, 2016. PLEASE NOTE: The White County Chamber of Commerce does not discriminate on the basis of race, color, age, sex, religion, national origin or against any organization. However, the Chamber of Commerce does reserve the right to refuse any entry that is inappropriate to the spirit of the event. Safety/Liability Due to safety considerations during the evening hours, entrants are prohibited from throwing items such as candy from floats to spectators. Walkers will be permitted to hand out candy, but should not hold up the traffic flow of the parade at any time. There will be no burning out, laying drag marks or any other form of aggressive vehicle behavior allowed. All violators will be removed from the parade and dealt with by the City of Cleveland Police Department. There will be no fueling of generators during the parade route or fuel cans (gasoline, fuel oil, etc.) allowed on board parade floats. There will be no live fires on any floats including wood stoves. There will be no vision obstructions to tow vehicles. An observer other than the driver is required with all towed entries. The observer can walk or ride. While every effort will be made to ensure that no accidents or injuries occur, the parade organizers and/or its agents are not responsible or liable for your entry or personnel at any time.

I, ______, hereby agree to hold harmless, indemnify and defend the City of Cleveland, its elected officials, its employees, the White County Chamber of Commerce, volunteers, agents or assigns (herein after referred to as CITY, CHRISTMAS IN THE MOUNTAINS, VOLUNTEERS AND RELATED PARTIES) from and against and all actions or causes of action for bodily injury, personal injury, property damage or wrongful death occurring or arising as a result of my participation in the Christmas in the Mountains Parade or any activities incidental thereto wherever or however the same may occur, and I, the named PARTICIPANT, do for myself, my heirs, executors, administrators and assigns hereby release, waive, discharge and relinquish any action or cause of action, which may hereafter arise for myself and/or for my estate, and agree that under no circumstances will I, my heirs or assigns prosecute or present any claim for bodily injury, personal injury, property damage or wrongful death against the City, Chamber of Commerce, Volunteers and Related Parties for any causes of action, except those arising from the sole negligence or willful misconduct of CITY, CHAMBER OF COMMERCE, VOLUNTEERS AND RELATED PARTIES. IT IS MY INTENTION BY THIS INSTRUMENT, TO EXEMPT AND RELIEVE CITY, CHAMBER OF COMMERCE, VOLUNTEERS AND RELATED PARTIES MENTIONED HEREIN, FROM ALL LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH EXCEPT FOR ANY LIABILITY ARISING FROM THE SOLE NEGLIGENCE OR WILLFUL MISCONDUCT OF CITY, CHAMBER OF COMMERCE, VOLUNTEERS AND RELATED PARTIES MENTIONED HEREIN.
I acknowledge and agree that any vehicle or tractor/implement I enter in the parade(s), as well as any vehicle or tractor/implement used to tow an entry will be covered by current automobile liability insurance and/or liability insurance that meets the State of Georgia’s requirements. I acknowledge and agree to abide by the Christmas in the Mountains Parade Regulations as printed on the second page of this application. I acknowledge that I have personally read, understand, and voluntarily sign this release and waiver of all liability and indemnity agreement, and am fully aware of the potential risk and hazards, which are inherent to participating in the Christmas in the Mountains Parade.
