1) _____ Become a student at CGCC. Visit the Admissions & Records Office at the Willams or Pecos Campus or online at: h t t p:// www . c g c. edu . Students must also create their account at My.Maricopa.edu.

2) ______Register for EMT 104 Class at Student Services at the Williams Campus, Pecos Campus or with Margie Zuccaro in Baylor South (WC) 480-988-8755. Pay course registration fee at either Pecos or Williams campus or use online payment fee option. You must make a form of payment within a day of registration or you will be purged.

______Provide a reading score to enroll in the EMT 104 3) Acceptable reading score documents are: a) Associate’s degree or higher. b) AIMS reading score of 720 or higher. c) Nelson-Denny of 10th grade or higher. d) Accuplacer reading score of 74 or higher. e) Compass reading score of 83 or higher. f) PSAT composite score of 93 or higher. g) SAT composite score of 930 or higher. h) Asset score of 42 or higher. i) Reading Course Placement Score.

(The above meet the requirements for EMT only, not a degree. Check with your advisor.)

4) _____ Register for and attend an EMT Orientation before the start of class. Attending an EMT Orientation is mandatory before attending class; however, you can register for class before attending an orientation. You can register for the EMT Orientation by going to cgc.edu, QuickLinks, click on E in the alphabet bar and click on EMT Program.

5) _____ For questions regarding your eligibility to be certified as an EMT in the State of Arizona, please call AZ DHS Bureau of Emergency Medical Services at (602) 364-3186. (Please note that CGCC is not authorized to answer any questions regarding this issue).

6) _ ___ Have a current CPR card for the duration of the semester. CGCC requires one of the following CPR certifications:

 American Heart Association: Healthcare Provider - American Red Cross: Professional Rescuer, or  Green Cross or NSC - Professional Rescuer, Military training Network-Meets AHA 2010 guidelines for Health Care Providers, or  American Safety and Health Institute: CPR Pro (CPR for Healthcare Professionals) O pt i o n1: E M T 1 04 AB EMT 104AB is an optional course designed to help prepare students for the national certification exam and provide simulated patient contacts in the pre-hospital setting. EMT 104AB students will be under the direct supervision of preceptor and must submit documentation of at least 10 patient contacts and a preceptor evaluation. Students selecting this option to meet the clinical requirement do NOT need a drug screen, flu shot, or a DPS fingerprint clearance card.

O pt i o n 2 : H o s p i t al E mer g e n c y D e pt. C l i n i c al Students may complete a scheduled clinical in an approved hospital emergency dept. Participating hospitals and requirements vary by semester; however, extensive online training modules will be required (up to 4 hours). A 12-panel drug screen, and a flu shot will likely be required.

Students must submit documentation of at least 10 patient contacts and a preceptor evaluation.

7) _____ Proof of a clean 9 panel drug screen within six months prior to scheduled clinical is required. Failure to provide proof of a clean drug screen will result in removal from EMT 104. Southwest Laboratories offers drug screening for a fee to CGCC EMT students. Check ht t p : // s o uth w e s t la b o ra t o ri e s .c o m/ for locations. Drug test forms will be handed out the first week of class. Do not go to Southwest Laboratories without the appropriate authorization form.

8) _____ Students must have or obtain a Level 1 Fingerprint Clearance Card. This is available through the State of Arizona Department of Public Safety. The fee for this service is $65.00 and usually takes between 6-8 weeks to process.

9) _____ Proof of Immunization for Rubella (German measles), Rubeola (Measles), and immunization (two shot series)

10) _____ Proof of Varicella shot or if you have had chicken pox, provide the age and year.

11) _____ Proof of a negative TB test (within six months prior to class)

12) _____ Proof of the Heptavax series – Hepatitis B (three shot series) or signed declination form is required.

13) _____ Read and hand in a signature page of the District Clinical Policy. (www.Healthcare.maricopa.edu).

14) _____ Proof of flu shot.

15) Purchase all necessary textbooks and equipment (Text book is available at the CGCC Pecos and Wililams Campus bookstores. Required text is: “Emergency Care and Transportation of the Sick and Injured”, 10th Edition, AAOS. ISBN: 9781284075311, Author: American . A stethoscope and penlight are also required for the class.
