Student/Parent Contract CJH Yearbook 2013-2014

Please review and complete the following contract. Return it to Mrs. Korff in the Counseling Center as soon as possible.

“To create the best working relationship between those involved, it is important that we agree on a given set of consistent expectations. Your initials and signature constitute agreement with this contract.”

INITIALS ______Parent * I understand that my student will need to devote time outside of the school day to complete assigned tasks. ______Parent * I understand that if my student misses class to participate in yearbook activities during the school day, he/she is required to complete any assignments missed by the next school day. I understand that it is my student’s responsibility to ask his/her teacher to provide the assignments ahead of time. ______Parent * I understand that my student must maintain passing grades in all of his/her classes in order to participate in any yearbook activities that occur during the school day. ______Parent * I understand that my student will be working with specialized equipment and supplies and will be held financially responsible for any damage and/or waste due to negligence. ______Parent * I understand that it is my responsibility to provide transportation or make transportation arrangements for my student to and from yearbook activities such as work days, photo assignments and any other activity related to yearbook. ______Parent * I understand that must student should be picked up no later than 3:30 pm when he/she attends weekly yearbook meetings.

I commit myself to making those times available when necessary. ______Student * I understand the importance of the equipment and supplies that I will be using and pledge to treat them with respect. Should I neglect and/or abuse the materials, I will repair or replace them at my expense. ______Student * I understand the importance of attending after school yearbook meetings. If I cannot attend, I understand that I should notify Mrs. Korff or Mrs. Lawson as soon as possible. ______Student * I understand that I am responsible for arranging transportation for after school meetings and other yearbook activities outside of school. I understand that I must be picked up no later than 3:30 pm on the afternoons of the yearbook meetings. ______Student * I understand that I must maintain passing grades in all of my classes in order to participate in yearbook activities that occur during the school day. ______Student * I understand that when I participate in a yearbook activity during the school day that I am responsible for getting any work I might miss in class from my teacher ahead of time and that I must complete the work by the next day. ______Student * I understand the importance of working together as a team and the importance of meeting deadlines. ______Student * I understand that I must be a positive representative for my school and work to produce and preserve the positive memories of our school.
