Dear Valued Patient:

In the very near future, you will receive information from the State of California about a new program which will require almost everyone who has both Medicare and Medi-Cal to choose to enroll in CalOptima or stay in traditional Medicare. If you do not make a choice, you will be automatically enrolled in CalOptima.

I am writing you this letter because I value our physician-patient relationship, and I would like to continue being your physician. But my ability to continue seeing you depends on choices you make in this transition.

Patients will begin enrolling into CalOptima in October of this year.

You will also have the option not to join a managed care plan, but you will have to make that choice. You will need to watch carefully for the notice from Medi-Cal, and indicate that you are opting out of the program.

Please know that I do not contract with CalOptima, so you must opt out in order to stay in my practice.
