<p>(Non-Contracting Physician: OC)</p><p>Dear Valued Patient:</p><p>In the very near future, you will receive information from the State of California about a new program which will require almost everyone who has both Medicare and Medi-Cal to choose to enroll in CalOptima or stay in traditional Medicare. If you do not make a choice, you will be automatically enrolled in CalOptima. </p><p>I am writing you this letter because I value our physician-patient relationship, and I would like to continue being your physician. But my ability to continue seeing you depends on choices you make in this transition.</p><p>Patients will begin enrolling into CalOptima in October of this year.</p><p>You will also have the option not to join a managed care plan, but you will have to make that choice. You will need to watch carefully for the notice from Medi-Cal, and indicate that you are opting out of the program.</p><p>Please know that I do not contract with CalOptima, so you must opt out in order to stay in my practice.</p><p>If you have any questions, please contact my office at ______.
