FIRST CONTACT KIT

Thank you for your enquiry regarding our children’s therapy & wellbeing services. Appointments are now being taken by phoning 6845 4494. Services will provided at Kidmotion headquarters from 9-4 on Tuesdays in Swift Street. In 2012 further appointment times will also become available.

We ask you to complete the attached First Contact Form and return to us BEFORE your first scheduled appointment. You may like to do this by opening the document, filling it out on screen, saving the document and returning by email. Alternatively, you can print it out and post back to PO Box 490 Wellington or slide it under the shopfront door. In completing the First Contact form please circle all responses that apply to your child MOST of the time. By using this form we can guarantee that even before your first appointment we are familiar with what motivates and challenges your child, so that treatment can commence more speedily.

Your child should come to appointments dressed in comfortable clothing (a tracksuit is perfect) with socks for gym work. Toilets are available and a kitchen for warming bottles, food etc. Parking is freely available outside the building, with 2 places reserved for cars with a disability parking permit. All services are within walking distance including the town park with playground, which is a lovely spot for lunch.

In order to maintain the privacy of our clients, the shopfront door is kept locked when therapy is in progress. If you arrive for your appointment and the door is locked, be assured we always do our best to run to time. The door will be opened in time for your appointment.

IF YOU ARE ACCESSING ANY OF THE MEDICARE OR FAHCSIA REBATES FOR YOUR APPOINTMENT PLEASE PHONE MEDICARE ON 13 20 11 BEFORE YOUR APPOINTMENT TO ENSURE YOUR GP/PAEDIATRICIAN REFERRAL HAS BEEN ACTIVATED. ALL PAYMENTS ARE REQUIRED AT TIME OF CONSULTATION AND YOUR REBATE WILL ONLY BE ACCEPTED IF YOUR REFERRAL IS ACTIVATED PRIOR TO YOUR APPOINTMENT.

Payment is to be made on the day of consultation. FAHCSIA clients excepted. HICAPS (instant Medicare and Private Health Fund rebates) and EFTPOS are available. Rebates: Complex Care Plan or Better Start for Disability referral from your GP. Helping Children with Autism plan referred by your Paediatrician. Kidmotion is a registered ancillary services provider with private health funds.
