Both Teams:

1. Complete your list of players for the fixture including their ID and shirt number. List these in alphabetical order. The completed match form should be handed to the umpires 15 minutes before the scheduled start time of the game. Put a star against the name of any bench player (if you only have 11 players none can be a bench player)

After the Game

Home Team:

1. Complete this form clearly and fully including entering any goals scored and their methods and any cards awarded for both teams and get it signed by both team captains/managers and umpires. Remind the umpires to mark on the sheet the reason for a card with either a P or D (see overleaf) 2. Submit the result of the match to the results service by text to 07976 601028 in the format homescore awayscore PIN (e.g. 4 2 1234) from your registered phone via the FixturesLive website (www.fixtureslive.net) The result should be notified by 6.30pm. If there is a problem with the results service telephone the result to the Match Secretary on 0870 760 6435. 3. Return this form to arrive no later than the Wednesday following the match to Caroline Hitchcock at Eastcot, Jolesfield, Partridge Green RG13 8JT. You must also scan in (both sides) and email the form to [email protected] the email must arrive by the required deadline. Any email must be “tagged” to automatically notify you when the email has been opened. It is your responsibility to check it has arrived in time.

Match sheet data will be verified each week by the league against the data entered into FixturesLive. Any failure to completely enter data or to deliberately enter inaccurate data is punishable by a fine and/or further sanctions. Please see Rule 8. of the league regulations for further information.
