SCIENTIFIC DIVE PLAN APPLICATION

All Dives must be planned around the competency of the least experienced diver. Before conducting any diving operation under the auspices of Texas A&M University, the Dive Supervisor for the proposed project must formulate a dive plan that addresses the following items below. This dive plan must be approved by a qualified Diving Safety Officer, one additional DCB member outside the Dive Supervisor’s department, and the University Scientific Diving Administrator (Director-Environmental Health and Safety Department. Dive plans can be formulated for extended periods not to exceed one year. If locations, techniques, environmental conditions or personnel do not change, additional dive plans are not required for the duration of the approved time period.

4). Attach a completed Diving Accident Management Plan which include the following items: a. Name, telephone number and relationship of emergency contact for each diver b. Nearest two operational recompression chambers and phone numbers c. Nearest two hospitals, distances to them and phone numbers d. Available means of transportation and phone numbers.

5. Divers and qualification relevant to the proposed dive plan. * Note: All Divers must have current diving accident insurance (DAN or equivalent). The Dive Supervisor is required to be certified as a Rescue Diver and be certified as an oxygen Provider by a nationally recognized diving organization.

6.) The Dive Supervisor shall maintain a copy of the approved dive plan application with an emergency action protocol plan at the dive site(s). Additionally, an Oxygen Kit and an approved First Aid Kit are required at the dive site(s). The Dive Supervisor is to initial this paragraph indicating an understanding and compliance with these requirements.
