Annual Review Procedures for Persons under the Responsibility of the District Committee on Ordained Ministry who are preparing for a future interview with the Board of Ordained Ministry

The following paperwork must be submitted to the DCOM Registrar FOUR weeks prior to your interview with the District Committee on Ordained Ministry. All questions are to be completed regardless of past interviews. If you have written on a subject for us in the past, please give fresh attention to the question so that we may see how you are growing.

1. Biographical Information Form 102. [form attached] A new Biographical form must be submitted each year.

2. A written report from your Mentor.

3. Your Mentor should accompany you to the interview.

4. A manuscript or full transcript of a sermon you have preached recently. With it please provide an MP3 recording in your email to the committee. If that is not possible, please send via regular mail one of the following: a CD or DVD of the entire service. (Audio tapes accepted if no other options available.)

5. A bulletin for the service in which the sermon was preached. Please do not include bulletin inserts.

6. A written report on your service & leadership in the church with reference to paragraph 324.2.

7. Give a brief written response to the questions in paragraph 324.9 g, j, & n of the 2016 Book of Discipline. Aim for clarity of your understanding. (A page per question should suffice.)

8. A description of your understanding of Baptism and how you celebrate Baptism with your congregation (1-2 pages).

9. A description of your understanding of Holy Communion and how you celebrate with your congregation (1-2 pages)

10. Reflect upon pastoral care, including your theology around pastoral care, and your practices (1-2 pages)

11. Reflect upon your understanding of three fold grace.

12. Your understanding of our connectional system. (Total of 2 pages)

b. Describe the function of Discipleship Ministries (https://www.umcdiscipleship.org/) Global Ministries (http://www.umcmission.org/) and Church and Society (http://www.umc.org/who-we-are/board-of- church-society) 1 paragraph for each.

c. How do you promote and support United Methodist Missionaries? (http://www.pen- del.org/missionaries) 1-2 paragraphs

d. Reflect upon the Congo Partnership and how it impacts (or could impact) your local church ministry. (http://www.congo-mission.org/about-us/) 1-2 paragraphs

13. Provide a written report on how you have worked on the recommendations from last year’s Annual Review interview.

14. If you are still in school: Submit most recent official transcript from your College, Seminary, or from the Course of Study.

15. Form 117, indicating how your home church is providing financial support for your candidacy.

ADDITIONAL IMPORTANT INSTRUCTIONS:

All written work should be reviewed by your Mentor before it is submitted to the District Committee on Ordained Ministry. For the glory of Christ and his Church, please give attention to grammar, punctuation, spelling, and the appearance of your document.

Please email all reports four weeks prior to your meeting with the district committee. They become part of your official District Committee on Ministry file. A sermon recording is preferred in an MP3 format that can also be emailed, but if that is not possible, please send via regular mail a copy of the recording.

NOTE: You are required to meet with DCOM yearly in order to have your license renewed.

DCOM will not accept faxed materials. Incomplete or late materials will not be accepted after the due date. Your interview will be rescheduled for the following year. If you are under appointment, this puts your appointment at risk.

Please use the enclosed instructions for creating a unified PDF file, and email this to the Dover DCOM. Be sure to title your pages so we know which page(s) answer which item requested above. Please also number your pages, and put your name on each page.

Please keep a copy of everything you submit for your personal records.
