Indonesian Street Festival (ISF) 2016 invites you to participate in this year’s celebration of Indonesian Independence Day. Those of you who participated in last year must know about the success of the event and we would like your participation to continue. The ISF 2016 will be held on East 68th Street, New York, NY 10065 (between Madison and Fifth Avenue) with anticipation of 25 vendors (food, product and non-profit organization).

HOW TO BECOME A VENDOR

1. Determine type of booth you need 2. Read Vendor Rules and Regulations very carefully 3. Obtain necessary licenses under vendor’s own name according to type of vendor. 4. Fill up the application form 5. Submit the application form along with vendor fee, copy of Photo-ID and copy of all necessary documents NO LATER by June 15th, 2016 to:

6. We will send a confirmation email once we receive your complete application and payment. VENDOR RULES & REGULATIONS

1. VENDORS: All vendors MUST have DCA 30-day Street Fair Vendor Permit from Department of Consumer Affairs. Aside from Street Fair Vendor Permit, vendors must show additional requirements: a. FOOD VENDORS  Tax ID  DOHMH Temporary Food Service Establishment (via Department of Consumer Affairs) or  Department of Health License  Detail description of the food (Name of food, Ingredients, and Pictures) b. HANDCRAFTS VENDORS  Tax ID  Detail description of the product (Name of product and pictures) c. NON-PROFIT VENDORS  Proof of Non-Profit Status  No product/service may be sold. d. BUSINESS or COMMERCIAL  Tax ID of Company  Detail description of product/service (Name of product and pictures)

2. BOOTH FEES: Booth size is 10’ x 10’ with 1 of 6 Ft table and 2 chairs provided by ISF Coordinator. Fees are listed on the front page of the application form and are due with the application. There are no refunds will be apply for any reason. 3. SALES: Products sold at ISF 2016 may be subject to advance approval. You will need to submit photographs and detailed descriptions of all goods intended to be displayed or sold in your booth. 4. SALES TAX: ISF 2016’s vendors are responsible for collecting applicable state sales tax and reporting all sales to the NEW YORK State Department of Revenue. 5. SELECTION: This is a non-juried show so unless otherwise informed by the street festival manager all vendors are accepted. Please note however management has the right to limit the number of vendors in each category (Food, Handcrafts, Non-profit organization, and Commercial) in order to have a well-balanced street festival. 6. HOURS OF OPERATION: All vendors must be open from 11am to 5pm on the day of street festival. No booth may be left unattended during these hours. 7. SET UP: Loading stuff for vendor will begin on Saturday morning at 09:45 AM . NO EARLY ARRIVALS ALLOWED IN. Check in is located at 5 E. 68th Street. NY - NY 10065 . Staff ISF Coordinator will be available to help vendors to locate their booth space. Vehicles parked in the street festival area between 08:00 - 10:30 AM will be impounded. You may bring your vehicle into the Street Festival area after check in for loading only , in no circumstances may you park or leave car unattended. All set up must be completed and vehicles removed from the streets by 10:30 AM SHARP Saturday morning for ISF Coordinator inspection. Please display your Coordinator Event Permit included in your check-in package for the ISF Coordinator inspectors. 8. TEAR-DOWN: Load out begins at 5:00pm on Saturday evening. All booths and vehicles must be cleared from the streets by 6:00pm. Contact the Vendor Director immediately if an emergency will prevent you from meeting this deadline. Failure to meet this deadline will result in a fine and impound of your property. 9. GARBAGE REMOVAL: Garbage removal from tear down will be vendor's responsibility to dispose and move it to designated location. 10. VENDOR STAFF: A vendor need to register all their employees' names who will be working on that day. They need to wear their own special uniform , hat , name tag and apron. For a security purpose no one allowed staying behind the counter without a name tag / uniform from their business owner. 11. SECURITY: YOUR BOOTH AND CONTENTS REMAIN YOUR RESPONSIBILITY. Neither the ISF Committee nor the City of New York assumes responsibility for injury to persons, or loss or damage to any property of the vendors, including theft. In submitting this application, applicant agrees to comply with all Festival, state and local regulations, and to save and hold harmless the City of New York , ISF Committee, their employees, officers, sponsors and agents from any loss or damage to any person or property caused by applicant’s operation in connection with ISF 2016, and further agrees to defend said Associations and City for any claims for such damage.

RESTRICTIONS: 1. Vendors are not allowed to “ROVE” with their merchandise at any time. 2. No booth may be left unattended during hours of operation. 3. No alcoholic beverages, illegal substances, pets, loud music or television and no “hawking” will be allowed in the vicinity of your booth. 4. Items promoting the use of illegal substances as defined by the Vendor Director will not be allowed. 5. No Smoking will be allowed in the vicinity of your booth and ISF 2016. 6. No MSG will be allowed to use on any Food. 7. All cloth, plastic or paper decorations or displays must be flame proofed. 8. All cooking appliances must be approved and tagged by the Fire Marshal's Office before such appliance shall be allowed" on the fair site. 9. No gasoline-fueled appliances shall be allowed. 10. All permits for cooking appliances including generator use shall be secured from the Fire Marshal's Office. 11. Propane appliances and allied equipment shall comply with PFB "Guidelines and Regulations for Propane Cooking Equipment". 12. Charcoal briquette or wood barbecues may be used provided fire is started early in the morning prior to the public being on the designated area. Absolutely no use of flammable liquids for fire setting. Ashes or clinkers from barbecues shall not be dumped on ground or into garbage receptacles. 13. All cooking appliances shall be on a sturdy base, stand or table to preclude their being tipped or knocked over. 14. A fire extinguisher of not less than a 2A-10BC UL rated is required at all locations where cooking is done or flame-producing devices are be* used- Deep fat fryers require a minimum 40 BC size extinguisher. 15. Styrofoam packaging CAN NOT be used to pack the food & drink. 16. The Glove, Hair net / Baseball hat , apron need to be wear all the time.

PENALTY Violation to above rules, regulations and restrictions is subject to lose of your deposit and additional penalty up to $500

LICENSE INFORMATION

1. NYS Certificate of Authority ( Tax ID #) . There is no charge for this. This can be obtained at : https://licensecenter.ny.gov/business-licenses 2. Food Protection Certificate. (check http://www1.nyc.gov/site/doh/business/health-academy/food-protection-online-free.page ) 3. Temporary Street Fair Vendor Permit . This cost $10/month ( Non Food Vendor ) & can only be issued to individuals with Tax ID # . 4. Temporary Food Service Establishment ( For Food Vendor ), the cost $70/year.
