ST ALBANS ROTARY AND SMUGGLERS’ NOTCH “2014 SEASON SKI AND RIDE WITH SKILL PROGRAM” INFORMATION SHEET FOR YOU TO KEEP

This program is for Grades 4 through 12 and will run for 10 weeks beginning January 4, 2013. The dates are January 4, 11, 18, 25- February 1, 8, 15, 22 – March 1 & 8.

Please Note: Due to the number of skiers at Smugglers’ Notch, there will be no lessons on January 18th (Martin Luther King Weekend) and February 15th & 22th (President’s Weekend/School Vacation)

The program will cost each participant $20.00 per week. Participants should bring the exact amount in cash. Smugglers’ Notch Season Pass holders will be charged $10.00.

The program includes an all-day lift ticket and 2 hour group lesson taught by a qualified instructor. Participants will be grouped on the first day in classes according to their ability. Lessons begin at 1:00 P.M. First time skiers or riders will attend 10:00 AM lessons beginning on January 4th. Please indicate on the form if your child is a first time skier/rider and if your child wants to take a lesson. NOTE: If your child’s ability level is in the 1-4 range, we strongly encourage them to take lessons. This is a great opportunity to learn as there is no additional cost for taking a lesson.

Participants must have skis or boards, properly installed and adjusted safety bindings and brakes, which are required by the resort. A certified ski technician must adjust the equipment. Skis and poles must be secured together to facilitate safe handling. Kevin Smith’s Sports Connection will have something available to help keep your skis and poles together at a very reasonable price.

Please make sure to thank them or anyone that works there, as without their generous contribution we would not be able to keep our participant cost as low as it is. *** **Note: To reduce confusion all equipment and bags should be clearly labeled with the skiers or riders name. The more the equipment is labled, the less likely it is to be lost or stolen!!! We strongly encourage that all children wear helmets.**

No skiers or Riders will be picked up on route. Swanton students may load at Grand Avenue Enterprises between 7:00 AM – 7:15 AM.

Registration forms should be turned in to: The St. Albans Recreation Department, located at St. Albans City Hall, 100 North Main Street, St. Albans. To be Pre-Registered forms must be dropped off no later than December 20 st , 2013. If you cannot get the form in by the 20th, just bring it on January 4th. Programs sponsored by Smugglers’ Notch Management Co., Inc., and the Rotary Club of St. Albans involve a variety of rigorous summer and winter recreational activities including but not limited to, warm ups, games, group initiative problems and numerous other rigorous adventurous activities. In completing the application and request form granting permission for______(name of participant) to participate, we understand that there are risks of physical or emotional injury or disability in participation in such activities and programs. We accept and assume all risks of injury or disability from participation and we RELEASE Smugglers’ Notch Management Co., Inc., and the Rotary Club of St. Albans, its employees, activity leaders and agents as well as the owners and agents of the facilities used for these activities for the program from any liability of any nature, including, but not limited to, any personal injury or property damage which may occur directly or indirectly from participation in program activities. I, the undersigned parent or guardian hereby agrees to indemnify Smugglers’ Notch and the Rotary Club of St. Albans from all claims, or suits including reasonable attorney’s fees resulting from involvement in the Programs at Smugglers’ Notch. I further agree that (1) any claim, of which I may at anytime bring for any reason against Smugglers’ Notch and the Rotary Club of St. Albans, its employees or agents shall be brought in state or federal court in Vermont and not in any other state or jurisdiction; and (2) any claim shall be governed by the laws of Vermont.

We understand that a further condition of participation is that each participant carry Health and Accident Insurance while participating in programming and hereby certifies that ______is covered under the following Health and Accident Insurance Plan:

In case of an emergency and Smugglers’ Notch and the St. Albans Rotary Club is unable to locate either of the parents or guardians and realizing that medical aid may be withheld from my child, I hereby authorize Smugglers’ Notch to seek medical aid for my child, as they deem necessary and appropriate.

I further agree to indemnify and hold St. Albans Rotary Club and Smugglers’ Notch Management Company, Inc., its representatives, agents, affiliates, officers, directors and employees harmless from any harm, injury, damage or loss to person or property to the aforementioned child. I also agree that if any portion of this agreement is determined to be unenforceable by a court of law, all other portions of the agreement shall remain in full force and effect.
