By signing this Foster Agreement, the Foster Home Applicant (hereinafter “Applicant”) agrees to the following rules and stipulations:

1. Applicant understands that placement of foster animals is at the discretion of the Petropolis Foster Coordinator, Board of Directors, and/or their designated representative, and recognizes that, while applicant has the right to refuse acceptance of any animal for any reason, any animal may be removed by Petropolis from a foster home for any reason at any time.

2. Applicant understands and agrees to care for the foster animal(s) in accordance with Petropolis guidelines by housing the animal inside Applicant’s own residence and providing a quality, commercial pet food, regular access to clean, fresh water, adequate exercise, and regular socialization; additionally

3. Applicant agrees to cooperate with Petropolis in the provision of adequate veterinary care, at Petropolis’ discretion and expense, in accordance with the Illinois Animal Welfare Act and the Illinois Humane Care for Animals Act.

4. Applicant agrees to house all foster animals separate from his/her personally owned animals and that he/she will maintain no more than four (4) foster animals in his/her home at any given time.

5. Applicant agrees to maintain sufficient records of any and all veterinary care, medications, treatments, etc. administered while in foster care and to furnish these records upon return of the animal to Petropolis.

6. Applicant agrees to cooperate with Petropolis regarding the showing of any foster animals to potential adopters, given reasonable advance notice and reasonable flexibility in terms of date and time.

7. Applicant understands that Petropolis and its agents, assignees, and/or designated representatives are not liable for any damage to Applicant’s property caused by the foster animal(s).

8. Applicant understands that Petropolis, its agents, assignees, and/or designated representatives are not liable for injury to or illness of Applicant’s personally owned animals caused by the foster animal(s), and is aware that Petropolis WILL NOT pay veterinary bills for such.

9. Applicant understands that Petropolis, its agents, assignees, and/or designated representatives are not liable for injury to applicant, his/her family, friends, and/or associates caused by the foster animal.

I have read and understand the above rules and stipulations and agree to abide by them for the duration of my appointment as a Petropolis Foster Home.
