Filled equations papers are to be submitted in a sealed cover superscripting “QUATIONS FOR CMO’s DRUGS AND CONSUMABLES PURCHASE (Emergencies) wit letter no/notification No on the top of the envelope. Quotations are to be submitted to the Chief Medical officer, RKS and Rourkela Sundergarh should contain following documents. 2. Preference shall be given to the ISO 9001 certified manufactures or their products. The Quotations will be received by the undersigned on or before up to 11 AM on dated 23.07.12 and quotation will be opened on same day i:e 28.07.12.at 1 PM. 3. The undersigned/committee is not bounded to accept the lowest tender rate considering the technical aspect, the rate of present tender will be valid up to 1 year or inviting of next tender whichever is earlier. 4. Valid TIN NO with tin clearance certificate. 5. Valid drug license issued by competent authority. 6. In case of distributer necessary letter of authorization of the manufacturer is to be enclosed. 7. In case of selection, the decision of technical expert committee is final. 8. In case of distributor the bidder is to submit a certificate from the manufacture; that quoted prices are genuine. 9. The purchase order will be communicated to the firm through mail of fax & firm shall be in a position to supply the medicines with in 28th JULY 2012 or else the earnest money will be forfeited. 10. The decision of the authority is final. 11. The goods shall be delivered at RGH Rourkela Sundergarh destination only. 12. The interested firm should be deposited Rs.10000 (Rupees Ten Thousand ) only through SBI bank Draft in favors of Chief Medical Officer RKS RGH Rourkela Sundergarh towards earnest money with the quotation paper which will be refund after finalization the quotation.
