Directions: Indicate your council's eligibility to be recognized as a gold or silver star council by checking the appropriate categories, and providing information as requested. All activities must have been conducted during the previous affiliation year (i.e. since last affiliation term).

To meet this requirement, there should be an increase in joint membership each fiscal year of between 5 and 10 percent. Councils with a joint membership of 501+ qualify as long as they have an increase of at least 25 joint members.
